Objective:To summarize the experienees of prosthetic hear tvalve replacement in elder patient, analysis the risk factor of perioperative, provide basis for clinical treatment. Methods:The data of 110 patients,underwent prosthetic heart valve replaeement in our hospital from January 2001 to Deceber 2009,were retrospectively analysed. To statictik and analysis the short-term effect of surgery patients, and analysis the risk factor by basic diseases, operation methods, extracorporeal circulation and aortic blocking time, and postoperative complications et al.Result:110 patients recovered 99 cases.There were 19 cases occurd complications, include 6 cases low cardiac output syndrome,2 cases ventricular fibrillation,4 cases respiratory failure,3 cases mutipul organ failure,4 cases sternal infection; 11 cases dead in hospital after operation, respectively is low cardiac output syndrome in 3 cases, ventricular fibrillation in 2 cases, respiratory failure in 2 cases, multiple organ failure in 3 cases,1 case sternal infection.99 patients who had recovered had the heart function improved, postoperative heart-echo found left atrial diameter, left ventricular diastolic dimension and systolic dimension, and pulmonary artery systolic pressure were decreased.The statistics results show that:preoperative left heart dysfunction, combined aortic and mitral valve replacement, prolonged CPB and aortic blocking, respirator auxiliary time long are the risk factors of early death after heart valve replacement.Conclusion:Proper indication,fully preoperative preparation, proper surgical techniques, better perioperative management and myocardial protection,proper postopera-tive treatment play important roles in achieving good results. preoperative left heart dysfunction, combined aortic and mitral valve replacement, prolonged CPB and aortic blocking,and prolonged respirator auxiliary time increase the short-time death of surgery.
