BackgroundsThe incidence of coronary heart disease (CHD) increased year by year. It has become a serious harm to human health.It has important value to assess the riskfactors for CHD in predicting the accurrence of cardiovascular events.HIF ((hypoxia inducible factor-1α, HIF-1α) is a DNA binding protein induced by hypoxia,it belongs to BHLH-PAS family of proteins.The study have found that myocardial ischemia and hypoxia can induce and increase the expression of HIF-1α,which could effectively protect the myocardium and also have a positive significance to improve myocardial ischemia and hypoxia.One of the target gene of HIF-1αtranscriptional activation is the heme oxygenase-1 (heme oxygenase-1, HO-1)—the key enzyme of heme decomposition, which can be broken down into carbon monoxide hemoglobin, bilirubin and ferritin,they play an important role in against ischemia-reperfusion injury, anti-inflammatory, anti-adhesion, inhibition of platelet aggregation,vascular smooth muscle cell proliferation and so on. HO-1 may be an important protective factor of CHD. ObjectiveTo observe the serum of HIF-1αand HO-1 levels in patients with CHD, and to analyze the relationship between those parameters and unstablility of coronary atheromatous plaque and the degree of coronary stenosis,And observe these protective mechanisms in myocardia.Methods82 patients visited the Department of Cardiology of the First Affiliated Hospital of Zhengzhou University from January 2010 to July 2010 were included. In accordance with the clinical manifestation,patients were divided into 4 groups:18 patients with stable angina pectoris (SAP) cases,31 patients with unstable angina pectoris (UAP), as well as 9 patients with acute myocardial infarction (AMI),and 24 patients as control subjects. All the subjects underwent coronary angiography. Except for acute myocardial infarction in patients with coronary heart disease, the subjects were divided into 3 groups according to the results of coronary angiography:single-vessel group (8 cases), double-vessel group (19 cases), more than three-vessel group (22 cases). The serum levels of HIF-1αand HO-1 from the elbow vein were determined by ELISA, and to analyse the correlation between serum HIF-1αand HO-1.Results1. In accordance with the clinical manifestation, the serum levels of HIF-1αand HO-1 in AMI group and UAP group were significantly higher than those in SAP group and control group(P<0.05).Though the serum levels of HIF-1αand HO-1 in AMI group were higher than those in UAP group,there were no differences between them(P>0.05).2. According to the degree of coronary artery disease, the serum HIF-1αand HO-1 levels were significantly higher(P<0.05) compared with normal control group. With the increase of lesions in branches of Coronary artery, the levels of serum HIF-1αand HO-1 gradually increased. And the levels in more than three-vessel group were significantly higher than the other groups(P<0.05).3. Correlation analysis showed that, the levels of serum HIF-1αand HO-1 were positively correlated (P<0.01).Conclusions1. In patients with coronary heart disease, serum HIF-1αand HO-1 levels were higher, which may be important in prediction of CHD.2. The extent of serum HIF-1αand HO-1 levels were positively correlated with each other. Increased serum levels of HIF-1αand HO-1 reflected acute inflammory response tomyocardial ischemia and is useful to define the unstabitity of plaque of coronary arteries in acute coronarysyndrome(ACS) patients.They are usefulness for risk stratification connect hs-CRP in ACS.3. With the increase of lesions in branches of Coronary artery, the serum HIF-1αand HO-1 levels were significantly increased,to some extent reflects the severity of coronary artery disease.
