Objective:①In normal side-lying position and different pillow's height condition, the three-dimensional finite element analysis of the cervical intervertebral disc of biomechanics. Expect that a pillow height, which can provide smaller cervical intervertebral pressure.②The pillow was applied to the clinical treatment of carotid cervical vertebrasickness. Through observational studies of carotid cervical vertebrasickness clinical symptoms and curative effect before and after the treatment to evaluated the clinical curative effect of treatment for carotid cervical vertebrasickness in using the pillow which its height is 18cm.Methods:③We chose one healthy adult male volunteer (Its height is 170cm,and its weight is 60kg),and conducted X-ray and clinical screening to eliminate cervical vertebra disease.The volunteer took side-lying position cushion pillow (We adjusted the pillow's height until the volunteer felt that the pillow's height was appropriate,and we recorded the pillow's height),then we used GE64 row helical CT to scan the neck evenly and the thick laye of scanning is 0.625 mm. We got the continuous fault CT images and saved the data with DICOM format.Then the CT image data were inputted to the Materialise Mimics which is three dimensional image processing software and then through editing tools extract the coordinates of the cervical boundary to product a three dimensional surface image of cervical spine.We reconstructed the cervical spine suiface model in use of magics software, meshing and optimizing.We exported the processed data with OUT format.We exported the data of the cervical spine model which was smoothed with STL format,and then the data of STL format was imported into the geomagic which is a kind of reverse engineering software to generate nurbs.The data of the nurbs was exported with IGES famat.Then the data of IGES famat was imported into the patran which is a kind of finite element analysis software to generate the closed surface.We used a similar method to establish disc finite element mesh model in the patran.Then we added main ligaments of cervical spine.We Setted element type and material properties,then constructed a three-dimensional finite element model of the cervical spine. We referenced the position of modle,after setting boundary conditions to constraint the seventh cervical,then we simulated five pillow height(that is 10cm,14cm,18cm,22cm and 26cm).Then we analysed of the cervical intervertebral disc of biomechanics in five cases.The results was used stress mises to display.②According to the case inclusion critetia,30 case of patients visited our hospital outpatient department were selected between Sept,2009 and Jan,2011.They were divided randomly according to the order of visiting into treatment group and control group,each of which had 15 patients.The treatment group adopted the method of acupuncture and adjustding the pillow height of side-lying position;While the control group adopted the method of pure acupuncture.The changes on clinical symptoms were observed before and after the treatments.The statistical software SPSS 16.0 for windows was used to analyze the aquired data.Results:1. When height of pillow is 10cm, the higher segments of stress distribution was roughly on cervical intervertebral disc of C3/4,C4/5, C6/7, the maximum stress value is 9.14Mpa, the minimal stress value is 0.218 Mpa, on cervical intervertebral disc of C3/4,C4/5, C6/7,its stress value range is 0.629-9.14MPa.2. When height of pillow is 14cm, the higher segments of stress distribution was roughly on cervical intervertebral disc of C3/4,C4/5,C5/6, the maximum stress value is 3.86Mpa, the minimal stress value is 0.071 Mpa, on cervical intervertebral disc of C3/4,C4/5,C5/6,its stress value range is 0.264-3.86MPa.3. When height of pillow is 18cm, the higher segments of stress distribution was roughly on cervical intervertebral disc of C4/5,C5/,C6/7, the maximum stress value is 1.94Mpa, the minimal stress value is 0.043Mpa, on cervical intervertebral disc of C4/5,C5/6,C6/7,its stress value range is 0.129-1.94MPa.4. When height of pillow is 22cm, the higher segments of stress distribution was roughly on cervical intervertebral disc of C3/4,C4/5,C6/7, the maximum stress value is 11.3Mpa, the minimal stress value is 0.34Mpa, on cervical intervertebral disc of C3/4,C4/5,C6/7,its stress value range is 0.783-11.3MPa.5. When height of pillow is 26cm, the higher segments of stress distribution was roughly on cervical intervertebral disc of C3/4,C4/5,C6/7, the maximum stress value is 18.3Mpa, the minimal stress value is 0.41 IMpa, on cervical intervertebral disc of C3/4,C4/5,C6/7,its stress value range is 1.26-18.3MPa.6. When the height of pillow increased from 10cm to 14cm, and incresed from 14cm to 18cm,the maximum stress value, the minimal stress value and its stress value range all changed from high to low. When the height of pillow increased from 18cm to 22cm, and incresed from 22cm to 26cm,the maximum stress value, the minimal stress value and its stress value range all changed from low to high.7.The curative effect comparison between both groups have statistical significance(P<0.01). The symptom scores comparison between both groups have statistical significance(P<0.05). In each group the symptom scores comparison have statistical significance(P<0.05)before and after the treatments.Conclusions:1. We concluded that the stress distribution of cervical intervertebral disc was the most reasonable, when the height of the man is 170cm,the shoulder width of the man is about 18cm and the pillow's height is about 18cm.2. When the height of pillow is 10cm,22cm and 26cm,the higher segments of stress distribution was roughly on cervical intervertebral disc of C3/4,C4/5 and C6/7. When the height of pillow is 14cm,the higher segments of stress distribution was roughly on cervical intervertebral disc of C3/4,C4/5 and C5/6. When the height of pillow is 18cm,the higher segments of stress distribution was roughly on cervical intervertebral disc of C4/5, C5/6 and C6/7.The high stress accelerated the intervertebral disc degeneration.We concluded that cervical spondylosis was the pathological changes of cervical intervertebral disc of stress concentration,and which was the center of pathological changes.3. The pillow which its height is 18cm has the effect of auxiliary therapy and improving clinical curative effect to the patient of carotid cervical vertebrasickness on the condition that the height of the patient is 170cm, the shoulder width is 18cm±0.5cm and usually slept in side position.
