Objective:To find out the impact factors of the patients after the heart valve replacements, analysis the relation between the variations of the special gene locus (rs2108622) of CYP4F2 and the warfarin usage of the patients, and provide the guidance of the reasonable and personalized medicine.Methods:1. Collect samples, given the heart valve replacements (implanting mechanic valves), totally 352, and collect their data and blood preparations. Record the usages of the patients when their INRs are up to the standard. Study the impactions of the genders, the ages, the operative techniques and the weights to the dosages.2. Use the method of polymerase chain reaction (PCR)-DNA sequencing to test the genotypes of rs2108622 for the samples, and definite the distribution of the genetic polymorphisms. Study the relation between the genetic polymorphisms and the warfarin usage.Results:1. The analysis to the factors which may impact the dosages in the general information:The daily dosage of 352 patients is (3.20±0.83)mg/day. The factors such as the genders, the ages and the operative techniques could not notably impact the dosages (p>0.05), however the weights could (p<0.05).2. The detecting results of CYP4F2 gene polymorphisms:Of the 352 patients,229 had homozygous (CC genotype),103 had heterozygous(CT genotype) and 20 patients had mutant homozygous(TT genotype), The allelic frequencies of C and T were 79.69% and 20.31% respectively.3. The results of correlational study of the SNPs of CYP4F2 and the warfarin dosages:The statistics shows that patients with 2 CC alleles require warfarin (3.07±0.79)mg/day, more than CT (3.41±0.85)mg/day and TT (3.63±0.92)mg/day. The univariate ANOVA find that there has statistical significance between the SNPs of rs2108622 and warfarin dosages (P<0.05).Conclusions:There could be found in the study that the factors such as the genders, the ages and the operative techniques could not notably impact the dosages, however the weights could. Moreover, the genetic polymorphisms of the cytochrome P450 (CYP) 4F2 could impact the dosages of the warfarin in different patients. Patients with TT of rs2108622 have more warfarin than CC, having the statistical significance. There should be important clinical significance of detecting the phenotypes of CYP4F2 of patients after the heart valve replacements to direct the use of warfarin.
