Objective:1.To study the rule of changes between syndrome differentiation type of TCM, plasma B-type natriuretic peptide and heart function index(NYHA classify, LVEF) in patients with chronic heart failure(CHF). 2.To research the relationship of changes between syndrome differentiation type of TCM ,Plasma B-type natriuretic peptide and heart function index(NYHA cardiac classification type,LVEF) in patients with chronic heart failure(CHF). Method: 84 cases who caught chronic heart failure were selected in Zibo Traditional Chinese Medicine Hospital. After collecting clinical symptoms, signs and related results, all the cases were divided into three groups: syndrome of qi deficiency and xue stasis, syndrome of phlegmatic flood obstructing lung and syndrome of yang deficiency with water flood. The data collected were underwent Chi-square test, One-way ANOVA and linear regression analysis. Results: 1.There are obvious correlation among syndrome type of TCM, the index of BNP and the heart function index (NYHA cardiac classification type, LVEF) in patients with chronic heart failure. 2.When the type of syndromes are changing among syndrome of qi deficiency and xue stasis, syndrome of phlegmatic flood obstructing lung and syndrome of yang deficiency with water flood, the indexes of BNP and NYHA cardiac classification type increase significantly and the index of LVEF decreases with statistical significance (P < 0.05); as for BNP index ,there existes obvious difference among the three syndrome types(P <0.01). Compared with syndrome of qi deficiency and xue stasis and syndrome of phlegmatic flood obstructing lung, the syndrome of yang deficiency with water flood elevates remarkably (P <0.01), while there is no statistical difference between syndrome of qi deficiency and xue stasis and syndrome of phlegmatic flood obstructing lung(P >0.05 ).3. Different NYHA grades contribute to the changes of BNP and LVEF) value significantly(P <0.01 or 0.05). With the increases of NYHA grade, BNP value goes up rapidly and LVEF go down clearly, but the BNP value is no statistical difference between NYHA gradeⅢandⅣ(P >0.05 ).4.THE linear regression analysis shows that the correlation between the index of BNP index and LVEF index was negative, but their relationship is not close and we can not use LVEF value to expect BNP value. Conclusion: 1.Heart function index can provide reference to classify syndrome differentiation type of TCM in patients with chronic heart failure(CHF).With the aggravation of type of TCM, the level of BNP and NYHA grade is developing gradually. 2. With the increase of NYHA grade, the level of BNP elevates correlately while the level of LVEF decreases conversely; the correlation between the index of BNP index and LVEF index was negative, but their relationship is not close and we can not use LVEF value to expect BNP value. |
