Backgrounds and ObjectivesEndometriosis (EMS) is a disease because of endometrial tissue growth in the other parts outside uterine cavity mucosa, From a cellular level and ends with dysmenorrhea and infertility as the main symptoms of persistent disease. Though endometriosis belongs to benign disease, it shows adhesion, aggression and angiogenesis as malignant biological behaviour, and looks like cancer. It seriously affects women's physical and mental health and quality of life. Since Sampson first reported the EMS In 1921, the EMS has become a common, frequently-occurring and modern disease of obstetrics and gynecology now. With the wide application of laparoscopic surgery, its incidence is still on the rise. The prevalence of endometriosis reaches to 10% in women of reproductive age, and approximately 25~35% of infertile women have EMS. Account for 40%~75% patients of endometriosis have dysmenorrheal. Despite its research deployed widely, But so far the etiology and pathogenesis are not completely clear. There is still valuable for its diagnosis and treatment research now, and cytokine research is now mainstream.. Adiponectin, a kind of specific protein (endogenous peptide or biological new protein), is newly discovered and secreted by fat cells. It takes functions through its special receptor AdipoRl, AdipoR2 and T-calcium mucin. According to current studies, adiponectin belongs to defensive collagen family members in human and has higher concentration in blood(5~30μg/ml). With age is irrelevant, also it do not be influenced by diet. Research proves adiponectin participating in except lipid metabolism process, anti-inflammatory medium, antiangiogenesis source, and anti fibrosis et al. It closely related to the pathogenesis of endometriosis. Always with its research mainly concentrated on diabetes, atherosclerosis, heart disease and insulin resistance to diseases, etc. As a kind of cytokines, the adiponectin research about endometriosis is few.IGF systems have a variety of biological activities. Studies have identified IGFS can promote cell proliferation and transformation, antiapoptotic and induce the formation of new blood vessels and lymphatic. Among them insulin-like growth factor-1 (IGF-1) is a research hotspot in cell biology branch discipline in recent years. Reports show both in situ and ectopic endometrium organizations of endometriosi, all have IGF-Ⅰexpress, and IGF-Ⅰexpress significantly in ectopic endometrium is higher than that in situ, and there is no difference in situ and normal endometrium. As Taketanit study proves that IGF-Ⅰis relevant to formation of ectopic lesions of endometriosis. So IGF-Ⅰmay have relation to the pathogenesis of endometriosis. Deeply study for IGF-I factor may help to elucidate the pathogenesis of endometriosis, and it may provide scientific theory for specific therapeutic methods.BMI (Body Mass Index BMI) is international commonly standard used to measure whether healthy and fat thin degree of body in endometriosis fields. Previous studies do not establish relationship between endometriosis and body mass index. Literature shows women with endometriosis often show thin shape whether in diagnosis or in the past time. Higher BMI has protective effect to the pathogenesis of endometriosis. However, the pathophysiological mechanism still is not clear, and needs to be studied further.This study aims to joint test the serum level of adiponectin and IGF-Ⅰin endometriosis, and calculates the corresponding BM. We investigate the significance of the expression levels of serum APN and IGF-I and correlations with BMI, to seek new point for further revealing the pathogenesis of endometriosis, and to reveal the pathogenesis about lower BMI of endometriosis. This kind of research has not been reported at home or abroad.Materials and Methods1. Experimental groups1.1. Experimental group:40 patients were chosen in the Second Affiliated Hospital of Zhengzhou University from November 2009 to April 2010. All of them, aged from 22 to 47, were confirmed for endometriosis after laparoscopic surgery and pathology, and the average age was 29.6 years old. According to The revised American fertility Society 1985(r-AFS)staging standard,Ⅰ~Ⅱ23 cases, andⅢ~Ⅳ17 cases.1.2. Control group:60 cases, aged from 21 to 51, were randomly chosen including 30 patients with other common gynecologic benign disease and 30 healthy-adult women who did physical examination in our hospital, and average age was 30.4 years old.1.3. Inclusion criteria:All the experimenter were not administered hormone drugs in half a year, and exclusion of endocrinologlst disease, without amalgamated uterine fibroids, adenomyosis, mammary hyperplasia and other hormone-dependent disease.2. Methods2.1. Testing method:Double-antibody sandwich enzyme-linked immunosorbent assay was used to assay the content of adiponectin and IGF-Iin serum., Data were recorded as average±Standard deviation.2.2. Statistical analysis software SPSS16.0 was used to analyze experimental data. Pearson correlation analysis tests was used to analyze the correlation between the expression levels of adiponectin and IGF-Ⅰin serum and BMI(α=0.05 was the significance level).Results1. The expression level of adiponectin in serum from endometriosis group was significantly lower than that of control group (P<0.01).2. The expression levels of IGF-Ⅰin serum from endometriosis group was significantly higher than that of control group (P<0.05).3. Between the common gynecologic disease group and healthy-adult women group, The serum levels of APN and IGF-Ⅰhave no significant difference (P>0.05)4. The serum level of APN of severe endometriosis (rAFSⅢ~Ⅳ) was significantly lower than that of mild endometriosis (rAFSⅠ~Ⅱ) (P<0.01).5. The serum level of IGF-Ⅰof severe endometriosis (rAFSⅢ~Ⅳ) was significantly higher than that of mild endometriosis (rAFSⅠ~Ⅱ) (P<0.01).6. BMI was significantly lower in women with endometriosis than that in controls (P<0.01)7. The BMI of mild endometriosis was higher than that of severe endometriosis(P<0.05).8. Between the two controls, The BMI has no significant difference (P>0.05)9. In endometriosis group, The BMI and serum level of APN existed a positive correlation (r=0.891, P<0.01), however the BMI and serum level of IGF-Ⅰexisted a negative correlation (r=-0.672, P<0.05), and there existed no correlation between the APN and IGF-Ⅰ.Conclusions1. APN and IGF-Ⅰmight play a role in the pathophysiological mechanism of endometriosis, There was no significant correlation between APN and IGF-Ⅰ.2. The APN may take inhibitory effects in endometriosis process.3. Women with endometriosis had lower BMI, which might be one of specific signs for endometriosis patients.4. The lower BMI of endometriosis patients might be relevant to the change of the serum levels of APN and IGF-Ⅰ.
