Comparison Of Therapeutic Effects Of Warming Spleen Decoction And The Kidney-Qi Bolus In Treating Yang-deficiency Syndrome Of Chronic Renal Failure

Objective:The study aims to using two treatments which are comprehensive analysis of disease and syndrome and simple syndrome differentiation (the Warming Spleen Decoction group and The Kidney-Qi Bolus group) and observing the changes of the clinical therapeutic indexes of the two groups to optimize the treatment of Yang-deficiency of both spleen and kidney syndrome of Chronic Renal Failure.Methods:Select 40 cases of patients are of Yang-deficiency syndrome of Chronic Renal Failure and divide randomly into A group of 21 cases and B group of 19 cases. Treat A group with Warming Spleen Decoction, treat B group with Kidney-Qi Bolus and observe for two periods that 3 weeks is a period. The major indexes for observing are the changes of the main symptoms, signs and renal function; the minor indexes are hemoglobin, thyroid function, creatinine and so on before and after the treatment.Results:(1) The A and B groups have absolute effect, the total effective rate of A group is 90.5%,the dramatic effective rate is 4.8%,and the effective rate is 52.4%,while the total effective rate of B group is 78.9%,the dramatic effective rate is 0%,and the effective rate is 21.1%.But comprise the two groups in comprehensive effect and syndrome effect,and the difference doesn't have a statistical significance. The results of the two groups before and after the treatment present that both A and B groups have dramatic effects on improving the symptoms of fatigue, shortness of breath and reluctance to speak, eating less and anorexia,soreness and lassitude in the loins and knees, intolerance of cold and cold limbs, lumbago and cold pain in the waist and copious clear nocturine. Besides, A group is better than B group in improving the symptoms of eating less and anorexia, intolerance of cold and cold limbs, and the difference has a statistical significance. B group is better than A group in improving the symptom of loose stools, and the significance has a statistical significance. Neither of the two groups has effect on improving the symptoms of heaviness in the body and limbs, swelling, pale complexion, no taste in the mouth and not thirsty, and the difference has no statistical significance. (2) A group can effectively lower the density of blood urea nitrogen, serum creatinine and increase the level of hemoglobin, and has a dramatic therapeutic significance statistically. B group can effectively lower the level of blood urea nitrogen, doesn't have a dramatic therapeutic significance statistically in lowering serum creatinine and increasing the level of hemoglobin. Comprise the two groups, A group is better than B group in lowering the level of blood urea nitrogen, serum creatinine and increasing the level of hemoglobin, and has dramatic difference, which indicates A group is better than B group in improving renal function and correcting anemia.(3) Most patients of Yang-deficiency syndrome present low level of T3,T4, and the two groups can improve the symptoms of Yang-deficiency to some extent after the treatment and adjust the level of thyroid hormone.(4)Uric acid and Uric creatnine are not influenced before and after the treatment and between the two groups.(5) A group and B group are under treatment for six weeks,and there is no adverse reaction.Conclusions:(1)Therapeutic effects of Warming Spleen Decoction and The Kidney-Qi Bolus treating Yang-deficiency syndrome of Chronic Renal Failure are definite, and the former are better in postponing progress of CRF than the latter. (2) Patients of Yang-deficiency syndrome of Chronic Renal Failure have low level of T3,T4, the two groups both can improve the symptoms of Yang-deficiency to some extent and adjust the level of thyroid hormone, yet the concrete function are undifinite.(3) Warming Yang and dispersing turbidity is a common treatment for Yang-deficiency syndrome of Chronic Renal Failure, and the treatment of comprehensive analysis of disease and syndrome will be an important research pattern of combination of traditional Chinese medicine and western medicine in treating chronic renal failure. Only master the right indications,dosages and periods,can the clinical application be safe and convenient,and easy to operate.
