The Impact Of Laser-assisted Hatching On Patients With Recurrent Implantation Failure After In Vitro Fertilization And Frozen/Thawed Embryos Transfer

Abstract/Summary:
OBJECTIVE:To determine whether laser-assisted hatching(LAH) can improve clinical outcome of patients with recurrent implantation failure after in vitro fertilization and frozen/thawed embryos transfer.Methods:One hundred patients with recurrent implantation failure were divided equally into LAH1 and control1 groups; Two hundred patients with frozen-thawed embryos were divided equally into LAH2 and control2 groups. On the day of embryo transfer, the zona pellucida of the selected embryos in the LAH groups were thinning by using laser system.RESULT(S):(1) In the patients with recurrent implantation failure, the rates of positive human chorionic gonadotrophin and clinical pregnancy in LAH1 group were 48%and 44%, higher than which were 30%and 28% in control1 group, but statistics compare no significant differences(P>0.05).The rate of implantation was statistically significantly higher in the LAH1 group as compared with those of the control1 group(25% vs 11.33%, P=0.003).The early abortion and twin pregnancy rates were similar.In LAH1 group have one ectopic pregnancy and one monozygotic twin pregnancy.(2) In the patients with frozen-thawed embryos, the positive human chorionic gonadotrophin and clinical pregnancy and implantation rates were similar for the LAH and control groups(50% vs 43%,P=0.321;37% vs 38%, P=0.884; 19.17% vs 16.50%, P=0.50 respectively).The early abortion and twin pregnancy rates were higher in the LAH2 group compared with the control2 group, but statistics compare no significant differences.In LAH2 and control2 groups each have three cases ectopic pregnancy, In LAH2 group have one monozygotic twin pregnancy. CONCLUSION(S):(1)The LAH improved the implantation rate in patients with recurrent implantation failure.(2) LAH did not improve the implantation rate of FET cycles with younger patients and should not be performed routinely in all frozen-thawed embryos at the cleavage stage.(3) Because sample size is small, whether LAH increased early abortion, ectopic pregnancy and monozygotic twin pregnancy incidence is not sure.
