The Effect Of Shensongyangxin Capsule On Serum Matrix Metalloproteinase-2 And High-sensitivity C-reactive Protein Levels In Atrial Fibrillation Patients

Objective:To observe the changes of serum matrix metalloproteinase-2 (MMP-2) and high se- nsitivity C-reactive protein (hs-CRP) levels before and after the treatment of shenson- gyangxin capsule in atrial fibrillation (AF) patients and the relationship of between MMP-2,hs-CRP and left atrial diameter.To investigate the effect of shensongyangxin capsule on the structural remodeling of atrial fibrillation.Methods:Seventy-two patients with AF were randomly divided into two groups:thirty-six patients were in the shensongyangxin capsule group(received shensongyangxin capsule 1.2g,tid,the period of treatment lasted 4 weeks) and thirty-six patients were in the control group(received routine theraphy only). Tweenty-six patients with sinus rhythm (SR) were included from hospital at the same time as SR group. All the patients were completed regular biochemical,electronic cardiography tests and color doppler echocardiography,et al. Measured serum MMP-2 and hs-CRP levels and LAD between SR group and AF group and between shensong group and control group before and after treatment.Results:1. The levels of the several indicators in SR group and AF group. There were no significant differences between AF group and SR group in gender composition, primary disease and NYHA heart function classification (all P>0.05). Serum MMP-2,hs-CRP levels and LAD were significantly higher in AF group compared with SR group. In AF group,with prolonging the duration of AF(paroxysmal AF group, persistent AF group and permanent AF group) serum MMP-2 and hs-CRP levels and LAD were gradually increased. Pairewisely compared, there were significant differences among the average of paroxysmal AF group, persistent AF group and permanent AF group. The AF group's serum MMP-2 level and LAD were positively correlated (r=0.748); the AF group's serum hs-CRP level and LAD were positively correlated (r=0.880); The AF group's serum MMP-2 and hs-CRP levels were positively correlated (r=0.888).2. The levels of the several indicators in the shensong group and the control group. There were no significant difference between two treatment groups in serum MMP-2 and hs-CRP levels and LAD before treatment. After treatment, serum MMP-2 and hs-CRP levels and LAD of the control group significantly decreased. After treatment, serum MMP-2 and hs-CRP levels and LAD of the shensong group significantly decreased. After treatment, the shensong group's serum MMP-2 and hs-CRP levels and LAD were significantly lower than the control group's. In the control group, serum MMP-2 and hs-CRP levels and LAD were also significantly lower in paroxysmal AF group,persistent AF group and permanent AF group before treatment compared with after treatment. In the shensong group, serum MMP-2 and hs-CRP levels and LAD were significantly lower in paroxysmal AF group,persistent AF group and permanent AF group before treatment compared with after treatment. In paroxysmal AF group,persistent AF group and permanent AF group, compared with the control group, serum MMP-2 and hs-CRP levels were significantly decreased in the shensong group after treatment; LAD also decreased, but there was insignificant difference.Conclusions:1. Serum MMP-2 and hs-CRP levels were higher in AF patients and serum MMP-2 and hs-CRP levels positively correlated with LAD,maybe atrial fibrosis and inflame- matory mechanisms participated in left atrial structural remodeling.2. Serum MMP-2 and hs-CRP levels were higher in AF patients and gradually increased with prolonging the duration of AF,maybe MMP-2 and hs-CRP were all the risk factors of delayed or inhibited AF.3. Shensongyangxin capsule decreased serum MMP-2 and hs-CRP levels and LAD,maybe improve atrial structural remodeling was one of pharmacologic mechanisms of shensongyangxin capsule.
