Objective: To investigate the causes and the prophylactic measures for the complications following correction of mandibular angle prominence.Method: A total of 60 cases were collected from 2006 to 2009. In these cases ,3 surgical procedures were performed respectively, angle splitting osteotomy ,wedge osteotomy and oblique curvy ostectomy . For the complications during and post operation ,analysis the reason and find the prophylactic measures.Results: All the patients are satisfied except one case with facial asymmetry and received 2nd procedure . Follow up time is between 3-24months,mean time is 12 months.Among the complications, 2 cases with the intraoperative massive hemorrhage. Afte facial artery ligation and pressure dressing ,bleeding stopped.4 cases with the intraoperative bleeding ,after electric coagulation,using hemostatics and blood transfusion, the patient got well afterwards. 3 cases with hematoma ,and recovered after debridement.1 case with inferior alveolaris nerve brocken, 1 case with the lip mucosa injury but recovered later.2 cases with paresthesia of lower lip,but recovered 2 months later.8 cases with distortion of mouth postoperatively,but recovered 3 months later.1 patient suffered pericoronitis of the wisdom tooth after extracting it ,and result in submaxilla abscess.The patient recovered after debridement and drainage.Conclusion: The prominent mandibular angle osteotomy is a dangerous procedure because of the complex anatomical structure near mandibular angle region.In order to avoid complication,some masseurs must be taken including ,choosing the suitable osteotomy method , extensive subperiosteum dissection ,reducing the masseter muscle injury , protecting the soft tissue around the mouth, appropriate pressure dressing and postoperative negative pressure drainage.
