Objective To investigate the advantages and the therapeutic effect of the deep inferior epigastric perforator flap in repairing the soft tissue defect of limbs,to summarize the feature and the value of the deep inferior epigastric perforator flap. Methods The literature that was about basic anatomical reseach and clinical applications for the deep inferior epigastric perforator flap was reviewed .From October 2005 to May 2009,53 casees with the soft tissue defects of limbs were reconstructed with the deep inferior epigastric perforator flap or free thoraco-umbilicus flap,25 casees used the deep inferior epigastric perforator flap and other 28 applyed free thoraco-umbilicus flap . Two groups were compared with the safety and followed up to observe the flap shape, sensory recovery and function of abdominal donor site, shape the situation. Results Experimental group, 24 flaps successfully survived, and 1 case underwent postoperative venous crisis, the probe was invalid and flap necrosis at last. The control group, 28 flaps survived completely. Patients followed up for 12 months to 60 months,all cases in DIEP group no abdominal distension or collapse occurs, no abdominal or back pain ,no abdominal muscle strength decline. Conclusions The deep inferior epigastric perforator preserve rectus abdominis ,minimize the damage to donor site and reduce the rate of complications.DIEP meets the direction of Micosurgical development ,is a promising flap.
