Objective To analyze the clinical characteristics of hip surgery of elderly patients in orthopedics department during preoperative period, and to explore the feasibility and value of the physiological function by surgical stress (E-PASS) scoring system which could predict the incidence of postoperative mortality and complications of aged patients after hip surgery in orthopedics department .Methods Retrospectively analyzing the clinical features of.186 elderly patients(≥70 years) after hip surgery in orthopedics department including components of illness, complications, surgery and anesthesia, postoperative complications and prognosis, etc. By comparing age, complications, and other differences between hip fracture group (Group A ) and hip surgery group (Group B),Using this system To evaluate the feasibility of E-PASS scoring system used in Group B. To predict the mortality and the incidence of complications with E-PASS scoring system in elderly patients after hip surgery, and to analyze the differences between predictable and actual value after operation. Results Most of elderly patients after hip surgery in orthopedics department are mainly (74.7%) hip fracture, often associated with one or more complications (76.3%) before operation, receiving general anesthesia (71.0%) in operation, and the joint replacement surgery was the main operation methods (68.2%). Except 3 patients died of heart failure and respiratory failure, others were safe and lived through the perioperative period. There were no significant differences between postoperative mortality/ morbidity rate of complications (2.24% / 30.10%) and the actual incidence rate (1.61% / 33.87%) with E-PASS scoring system, O / E values were 0.75 and 1.13 respectively.Conclusion E-PASS scoring system could predict postoperative mortality and morbidity rate of complications of elderly hip surgery patients simply and effectively,which is useful for the preoperative avoidance or for adjusting risk factors and which was significant to reinforce perioperative management and to help patients smoothly through perioperative period.
