Study On Improving Hospital Compensation In County-level And Above Of New Rural Cooperative Medical System

Objective:This study analyzes the level of hospital compensation and factors of the actual compensation of county hospital in Hubei Province,in order to Increase the actual compensation of county hospital, and to promote sustainable development of the NCMS.Method:For achieving the research objective and insure the research quality, this research used the Quantitative Study and Qualitative Study as the main methods. This method include:(1) literature study: Use the network, books, journals and other tools to consult the literature and data to have a comprehensive understanding of the compensation scheme of Health insurance. (2)Site investigation. Through spot field investigation, interviews and other methods, we study the Compensation /situation and development of NCMS in the Survey area. .(3)Statistical analysis methods: use the descriptive analysis to analysis data.(4)The theoretical analysis method: use the principles - agent theory,the public finance theory. And through theoretical reasoning methods to put forward suggestions to improve the actual compensation of county hospital of the NCMS.Results:(1) lower levels of hospital compensation in county-level and above affect the overall level of compensation。The actual compensation for township medical institutions have reached a high level,so it is difficulty to continue to improve much more higher.(2) Hospital rates and hospital costs are rising.there is a weak in Medical institutions. With the continuous development of social economy, medical needs of farmers are increasing, the rapid rise in hospitalization rates are adding to pressure on the fund.(3)there are weaknesses in health care management. the patient flow is unreasonable .On the one hand, the income of farmers is increaing, many people wants to achieve better health services, on the other hand, two-way referral system is not perfect, Rely solely on economic factors to guide the flow of the method of its role of farmers is extremely limited.(4) Lower compensation of major diseases affects the overall level of compensation. because most of these patients are treated in higher medical institutions, while the proportion of compensation set low. this way, the lower compensation Not only is the heavy financial burden on families, but also affected the overall level of compensation(5) the low compensation and complex reasons of accidental patients is affecting the overall level compensation. Accidental patients take a lot of funds, and there are not uniform standards and reasonable compensation ratio of accidental patients ,so it is affecting the overall compensation.Suggestion:(1) We should improve the overall compensation level in ensuring the safety of the funds and expand the range of moderate and set from the pay line reasonably,(2) regulate the two-way referral system strictly and explore the first diagnosis system Actively, allocate medical resources rationally, and strengthen the service capacity of primary health care institutions to enhance the general medical education(3) Control the high cost of hospitalization cases, Explore the standard of accidental injury.(4) Explore the reform of payment and Strengthen management of medical institutions and control the unreasonable medical expenses of inpatients.
