Objective:To study the microsurgical operation by unifrontal interhemispheric approach for part of the anatomical factors of this approach and the surrounding anatomic structures adjacent the anterior communicating artery complex that need attention. to observe and record and measure the data during the exposure of the anterior communicating artery complex, provide anatomical basis for surgery. Methods:Fully fixed wet cadaveric head specimens of 15 chinese adults by 10% formalin, injected with red latex solution in the common carotid artery, and blue latex solution in the internal jugular vein. then odor removal, softening and bleaching, simulate the microsurgical operation by unifrontal interhemispheric approach,measuring the maximum angle between the long axis of the skull base and the Imaginary line that can expose the anterior communicating artery,the nearest point from the anterior communicating artery to the nasion point,and their perforating branches, recording the number of the perforating arteries and the Heubner arteries. measure the various forms and variations of the blood vessels, nerves, blood vessels under the microscope. recorded and photographed.Results:By the former interhemispheric approach to exposure of anterior communicating artery complex, the maximum angle between the long axis of the skull base and the Imaginary line that can expose the anterior communicating artery is between 15.36~28.54°, the shortest distance from the outer edge of the skull of the nasion point to the anterior communicating artery is between 48.52~58.86mm,the distance from the intersection point of the imaginary line which can reveal the anterior communicating artery complex and the frontal pole to the nasion point of 31.48~54.26mm, respectively, mean 22.45°,53.35mm,40.23mm, the data of the anterior communicating artery complex has no significant difference with which had reported in reference, no significant anatomical variation.Conclusions:the surgical operatons which need the interhemispheric approach especially the operations need the exposure of the anterior communicating artery complex, such as the anterior communicating artery and corpus peripheral arterial aneurysm surgery, craniopharyngioma located at the anterior third ventricle and the pituitary tumors far on the top of the saddle need to pay attention to the size of the craniectomy, the most suitable angle and the depth of the surgical approach and the variation of the anterior communicating artery complex, this experiment can provide some of the anatomical basis data.
