Objective:To study the effect of mild hypothermia combined with TP-5 to T lymphocyte subsets and prognosis of patients with severe traumatic brain injury. Metheds:All 42 patients with severe traumatic brain injury (Glasgow coma scale score of 8 or less on admission) were randomly divided into the mild hypothermia combined with TP-5 group [experimental group,22 case] and hypothermic group [control group (一),20 case]. Meantime, giving TP-5 with intravenous drop infusion and conventional therapy, hypothermia was induced within 24 hours of injury.The therapeutic regimen of control group (一) was the same with experimental group except for TP-5.Experimental group and control group (一) were detected with T lymphocyte subsets (CD3+,CD4+,CD8+,CD4+/CD8+) of venous blood on days 1,3,7,14 post injury by flow cytometry. The healthy check-up [control group (二),18 case] were detected with T lymphocyte subsets (CD3+,CD4+,CD8+,CD4+/CD8+)of venous blood by flow cytometry.T lymphocyte subsets were compared between the three groups. Hospital onset of infection and the prognosis of the patients were observed between experimental group and control group(一).Results:On days 1 post injury, T lymphocyte subsets(CD3+,CD4+,CD8+,CD4+/CD8+) didnt have difference between experimental group and control group(一); T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+) have difference between experimental group and control group(二); T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+) have difference between control group(一) and control group(二). On days 3 post injury, T lymphocyte subsets(CD4+,CD4+/CD8+)have difference between experimental group and control group(一); T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+)have difference between experimental group and control group (二); T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+)have difference between control group(一) and control group(二).On days 7 post injury, T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+) have difference between experimental group and control group(-); T lymphocyte subsets(CD3+,CD4+,CD8+) didn t have difference between experimental group and control group(二); T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+) have difference between control group(一) and control group(二).On days 14 post injury, T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+) have difference between experimental group and control group (一); T lymphocyte Tsubsets(CD3+,CD4+,CD8+,CD4+/CD8+)didn t have difference between experimental group and control group(二); T lymphocyte subsets(CD4+,CD4+/CD8+)have difference between control group(一) and control group(二).There was no difference between experimental group and control group(一) in the increased ICP. There were difference between experimental group and control group(一)in hospital onset of infection. There were difference between experimental group and control group(一) in the prognosis of patients.Conclusion:Mild hypothermia combined with TP-5 could improve T lymphocyte subsets of patients with severe traumatic brain injury, decrease hospital onset of infection rate and improve the prognosis of patients. Mild hypothermia combined with TP-5 can was safe and effective to patient and it has potential clinical application.
