Objective: Investigate left ventricular(LV)systolic function radial strain and twist in patients with essential hypertension (EH) during stage 3 by using two-dimensional speckle tracking imaging,probe into the influence of LV twist remodeling on cardiac systolic function and the value of twist.Methods: According to left ventricular ejection fraction(LVEF) exists decreasing whether or not.The 50 patients with EH during stage 3 were divided into two groups. EH groupⅠ:27 cases of left ventricular systolic function did not decrease (LVEF≥50%);EH groupⅡ: 23 cases of left ventricular systolic function decreased(LVEF<50%).Two dimensional images were recorded from the LV short-axis view at the levels of mitral annulus, papillary and apical for every EH group and 25 cases normal pattern group.Using 2D-STI,we measure and calculate the left ventricular peak systolic radial strain,the peak of left ventricular apical horizontal rotation angulation, the peak of the base of heart horizontal rotation angulation and the peak of left ventricular twist angulation.Results: Compare the parameters of radial strain:the curve of the cardiac systolic radial strain is consistent between hypertension group and normal pattern group.It is systolic unimodal forward curve.In EH groupⅠ,the peak of systolic radial strain was lower than that of normal pattern group at the levels of papillary,but the cardiac systolic radial strain was the same as normal pattern group at the levels of mitral annulus and the apical.In EH groupⅡ,the peak of cardiac systolic radial strain were lower than that of normal pattern group and EH groupⅠat the levels of mitral annulus, papillary and apical (P﹤0.05).Compare the parameters of LV rotation and twist:In EH group I,apical horizontal,heart horizontal and LV twist rotation angulation were increased than that of normal pattern group(P﹤0.05).In EH groupⅡ,apical horizontal,heart horizontal and LV twist rotation angulation were more lower normal pattern group and hypertension group I(P﹤0.05).Conclusion: The local myocardial function has been damaged in patients with staging 3 essential hypertension when left ventricular systolic function is in normal.It's the end of systolic function handicap when LVEF < 50%.Two-dimensional speckle tracking imaging is the new findings for the investigation of part or whole cardiac muscle motion.The investigation of cardiac muscle twist and cardiac systolic radial strain gives an explain in reason to left ventricular remodeling on the level of cardiac muscle from mechanics in hypertension,and provides theoretical bases for cure and prognostic evaluation of hypertension and heart disease.
