Analysis Of The Problem Related With Endovascular Repair Treatment Of Abdominal Aortic Aneurysm

Objective To study the Clinical curative effect, the surgical treatment and the methods to deal with the complications, to investigate clinical experience of EVAR and to guide the clinical work.Methods During the March 2009 to March 2001, all of the 21 AAA apatients was performed with Endovascular repair, including 19 cases of male and 2 cases of femal, mean age 61 years, range 40 to 82 years. All of the cases before the operation with the CTA,MRA or DSA, to measure those,(1)the diameter of Neoplasia neck, the maximal diameter of AAA. (2) the diameter of Common iliac artery and External iliac artery. (3)the length and the included angle of proximate neoplasia neck. (4)the length of AAA. (5)the length of Common iliac artery. Based on those datas, to choose the suitable Stent-graft: 1 cylinder Stent-graft, 20 bifurcate Stent-graft with 15 one-piece Stent-graft and 5 two-piece Stent-graft. The operation working under the DSA, when determine the precise location of the Stent-graft, open the Stent-graft. To broaden the Stent-graft with sacculus proprius or put a cuff when there is endoleak. Tollow up the patients with CT or Doppler after operation to observation the AAA and Stent-graft.Result After EVAR, immediately with DSA, the result showing: 15 cases aneurysms disappear, without endoleak appearing in the both ends of Stent-graft; 5 cases aneurysms disappear, with small endoleak, 1 cases with visit, 4 cases after giving a ball bag extend the endoleak disappearance; 4 cases with the iliac-femoral artery Serious stricture disappear, and in the Stent-graft, the blood flow to be unobstructed; 10 cases with both iliac aneurysms after putting cuffs, the aneurysms disappear. The patients after operation was equability. When the patients leaving hospital, the pulsatory tumor disappear, 14 cases abdominal pain disappear. The CT or DSA one year after EVAR with visit to show to the Stent-graft didn't move and had no endoleak; 18 cases pseudocoele disappear; 2 cases AAA don't increasing; 2 cases with Endoleak disappeared by putting Cuff; 4 cases without having or ease the lower limb symptom of ischemia after EVAR. All of the cases without infarction of kidney, limping of fundament and colonic ischemia.Conclusion Endovascular repair treatment of Abdominal aortic aneurysm with the small wound, instauration quick and having good curative effect in the near future.
