Objective To explore the clinical characters and surgical strategy of intracranial giant aneurysms.Methods Collected clinical data and follow up outcome of 28 consecutive patients with intracranial giant aneurysms from January 2007 to January 2011 were analyzed retrospectively and summarized the surgical treatment experience. All patients are treated with surgery .The improved operative techniques were applied including the temporary proximate and distal occlusion of parent artery, stepwise moulding of aneurysms neck, sucking and decompressing the body of aneurysms, and resection of aneurysms body, et al. surgical results were evaluated by the Glasgow Outcome Scale (GOS).Results The 28 patients underwent neurosurgery including resection of aneurysms body in 12 cases , direct clipping of the aneurysms and reconstruction of the parent arteries in 9 cases, proximate occlusion of parent artery in 1 and trapping aneurysm in 6. According on Glasgow Outcome Scale on discharge from hospital of 28 patients,20 were recovered well, 4 moderately disabled, 3 severely disabled and 1 died. The fair cure rate was 71.43% and the total mortality was 3.57%.Conclusions Surgical treatment is an effective treatment for intracranial giant aneurysms, the aggressive and individually surgical treatment should be considered for it. The appropriate surgical approach can improve the clinical outcome and reduce the postoperative complications obviously.
