Objective:To study the position and its changes of clicking temporomandubular joint (TMJ) disc, before and after it was cured by conservative therapy, and to study the relation between clicking and the position of articular disc.Methods:1. Objects:19 TMJ clicking patients 27 sides TMJ were selected as group A1, and when they had been cured for 3 months with no clicking as group A2; other 5 volunteers with no clinical symptom 10 sides TMJ were selected as group B.2. MR examination:Using MR machine, both TMJ sides of group A1,A2 and B were scanned and imaged in the intercuspal contact position (ICP), with the established optimum scanning method.3. Image analysis:The position of articular disc to condyle was analyzed quantitatively in the scanning procedure at the position of oblique sagittalia plane. And the angle between articular disc and ventricular line on 12 o'clock site of condyle as measured, as the position of articular disc. What's more, the displacement positive number and negative number of every group were noted.4. Conservative therapy:The system functional exercise was chosen.5. Statistical analysis:Compare the positions of articular disc of three groups and then do analysis of variance; compare the anterior disc displacement positive and negative number of three groups and then do chi-square test.Results:The sharp MR imaging of all objects had been got. Analytically, the articular disc position of group Al was 32.6000±9.93014, and the positive rate of anterior disc displacement was 100%; the articular disc position of group A2 was 20.5704±7.56240, and the positive rate of anterior disc displacement was 92.6%; the articular disc position of group B was 8.8900±6.04013, and the positive rate of anterior disc displacement was 30%.1. Comparing group A1 and B, the result showed that the articular disc position of group A1 was more obviously anterior than that of group B, and the positive rate of anterior disc displacement of group A1 was higher than that of group B.2. Comparing group A2and B, the result showed that the articular disc position of group A2 was more obviously anterior than that of group B, and the positive rate of anterior disc displacement of group A2 was higher than that of group B.3. Comparing group A2 and Al, the result showed that the articular disc position of group A2 got obvious revival, but the positive rate of anterior disc displacement of group A2 got no reduced than that of group A1.Conclusions:The articular disc position of patients suffering TMJ clicking was more obviously anterior than that of volunteers with no clinical symptom, and the positive rate of anterior disc displacement of patients was higher than that of volunteers. Treated with conservative therapy, the articular disc position of patients got obvious revival, but also was more obviously anterior than that of volunteers; the positive rate of anterior disc displacement of patients got no reduced than before, but higher than that of volunteers.
