Objective:To evaluate the relation between the atrial conduction interval and Angiotensin II receptor blocker by Tissue Doppler Imaging to reseach the possible mechanism of these drugs to reduce atrial fibrillation. And to discuss the application for TDI in cardiac electrophysiology research.Materials and Methods:In study we select 60 hypertensive inpatients or outpatients accorded with the inclusion in Shandong Taishan hospital, they were randomly divided into Irbesartan group(30 cases)and amlodipine group(30 cases) according to occupation, gender, age, weight, height and so the principle of consistency, meanwhile 30 patients were selected as the control group according to the same principle with normal blood pressure. Irbesartan group was treated with oral irbesartan, the initial dose of 150mg, once daily, if necessary, increase the dose. Amlodipine group were treated with oral amlodipine to control blood pressure, when necessary, two groups w plus diuretics, two groups target blood pressure were systolic blood pressure<140mmHg and diastolic blood pressure<90mmHg, atrial conduction intervals were measured by Tissue Doppler Imaging medication before and 6 months after medication. Datas were analysised by SPSS statistical software.Results:(1) there were no statistically different between irbesartan group and amlodipine group in age, systolic blood pressure, diastolic blood pressure, duration of hypertension. (2) irbesartan and amlodipine groups before treatment have no statistically difference in the atrial conduction interval, but they all have statistically difference with the control group. (3) irbesartan group atrial conduction interval has significant statistically change before and after medication. But the left atrial size has no significant statistically change before and after medication. (4) amlodipine group before and after treatment, there were no statistically difference.Conclusion:1. abnormalities are existed in hypertensive patients atrial conduction interval.2. the atrial conduction interval of the irbesartan group after treatment is shorter than before treatment, amlodipine group, no significant change before and after medication, But the left atrial size of the irbesartan group has no significant statistically change before and after medication. indicating ARB has effect on intra-atrial influence (which can speed up the intra-atrial).3.TDI is a good method to the study on the impact of drugs to cardiac electrophysiology in non-invasive ways.
