Objective:Heart failure(HF), it is a cardiac dysfunction syndrome caused by all kinds of heart disease. With the aging of the population in the world, its prevalence is increasing year by year. In recent years, studies shows that the most important mechanism of heart failure is the cardiac reconstruction and reinforce with the development of heart failure each other, myocardial cell apoptosis and myocardial fibrosis are important pathophysiological changes in the remoding of heart. But its pathogenesis is not fully clear. Telomere is the repeat DNA sequence at the end of chromosomes, it has protective effect to the end of the chromosome,and it is a kind of important mechanism about cell aging and apoptosis. Research has shown that telomere shortening have correlation with the pathogenesis of heart failure. This subject is to investigate the relationship between telomeres and heart failure and its role in two important aspects heart reconstruction myocardial fibrosis and the role of myocardial cell apoptosis.Methods:60samples of cardiac muscle, get from heart surgery patients of our hospital, are divided into two group,30cases each, cardiac function normal group30example; Hf group30example.To acquire a little tissue of right ear atrial of patients and get the peripheral anticoagulant1ml and not anticoagulant2ml in the process of the heart surgery. by using real time fluorescence quantitative (FQ-PCR) to detect telomere length of the peripheral white blood cells; By using the method of ELISA to test the serum concentration of P I CP, PIIINP of these people above.Intend to do the correlation analysis with telomere length,heart function classification and age. Myocardial tissue pathology after HE dying to observe myocardial fibrosis. Using Tunel method to study myocardial cell apoptosis.Result:(1) compared with the normal heart function group, HFgroup telomere length of peripheral blood leukocyte significantly shortened (P <0.05); The correlation of telomere length and cardiac function classification present negative correlation (r=0.423P <0.05); the telomere length of the two groups is negatively correlated with age, including HF group (r=0.639, P <0.05), the nomal heart function group (r=0.614, P <0.05); The telomere length of HF group and serum P I CP, PIIINP concentration is a significant negative correlation (r=0.678, P<0.01; r=0.815P <0.01);(2) compared with the normal heart function group,the concentration of serum P I CP, PIIINP increased significantly in the HF group patients (P<0.01, P<0.01); The correlation of serum P I CP, P III NP content and age in the two groups are positively related (r=0.678. P<0.01; r=0.458, P<0.05); Serum P I CP, P III NP content and cardiac function classification are positively related (r=0.41, P<0.05; r=0.449, P<0.05);(3) peripheral blood leukocyte telomere length in the heart failure patients are independent, significantly negative correlation with serum P III NP after correlation analysis(P<0.01);(4) pathological HE dyeing of the two groups demostrate:there is a large patch of collagen deposition in the heart failure patients, a few collagen deposition in the nomal heart funcion group;(5) myocardial apoptosis index are positively related with heart function classification, r=0.913P<0.01; Apoptosis index are positively related with age.Conclusion:(1) Peripheral blood leukocyte telomere length of HF group patients is shorter than that of nomal heart function group, telomere length is negtively correlated with heart function classification, shortening of telomere participate in the occurrence of heart failure.(2) concentration of serum PICP and PIIINP in HF group is higher than those of nomal heart function group, serum PICP and PIIINP concentration was positively related with cardiac function classification, HE dyeing slice tip:hf group of myocardial fibrosis degree is not clear hf group, the levels of serum collagen concentration is increased with cardiac function classification; myocardial apoptosis index of Hf group is significantly greater than nomal heart function group, there is significant differences between the two groups; Apoptosis index is positively related with cardiac function classification; thus demonstrate that myocardial fibrosis and myocardial apoptosis is an important mechanism of the heart failure, and myocardial fibrosis and myocardial apoptosis involved in process of heart failure.(3) telomere length in HF group is negtively related with serum PICP, PIIINP concentration, shows that telomeres length is associated with myocardial fibrosis;(4) telomere length is negtively related with age; Two groups of collagen content is positively correlated with age. the older, the highe of the serum PICP and PIIINP content; Myocardial apoptosis index is also positively related with age, the older the patient, the higher the myocardial apoptosis index, the two important aspects of the remolding heart:myocardial fibrosis and myocardial apoptosis is associated with the aging process, aging is that the independent factors of heart failure.
