[objective] To observe the change of liver function before and after transjugular intrahepatic portosystemic shunt (TIPS), and discuss the acting mechanism and influencing factors. TIPS is observed before and after the treatment of cirrhotic and portal hypertension to its influence on liver function.[methods] To use TIPS cure bleeding from esophageal varices or (and) refractory ascites because cirrhosis and portal hypertension in patients with TIPS therapy, follow-up visit Hepatic function tests main index change after postoperation three days, twoweeks. one month, three months, six months, twelve months.[results] TIPS cures patients with cirrhosis and portal hypertension114case(male/female81/33),age from15to X4years old, mean age51.871:14.13years old, liver function grade Child-PughA20case. Child-PughB57case, Child-PughC37case. TIPS success rate Is100%,101cases in follow-up periods from3-24months (mean12.25month),89case survival,25case death, statistic analysis Hepatic function tests main index, there's no statistically significant differences compared Hepatic function tests main index change after TIPS three days, twoweeks, one month, three month, six month and one year with preoperation.[conclusion] TIPS is the treatment of cirrhotic and portal hypertension complications such as gastrointestinal bleeding, intractable ascites, Budd-Chiari syndrome, interventional radiology therapy. It can relieve portal hypertension effect rapidly and significantly, but it may also bring some patients with complications, such as hepatic encephalopathy, shunt tract obstruction or occlusion, hepatic damage. After the operation due to iatrogenic mechanical damage and portal vein shunting, short-term inside liver damage. But, in the long term, less influence is on liver function.
