Objective: This study aimed to explore the impact of surgical resection of the prognostic factors ofpatients with hilar cholangiocarcinoma. Methods: Retrospective analysis from January2002to December2011admitted to the First Affiliated Hospital of Fujian Medical University during generalsurgery line clinical data of55cases of surgical treatment of hilar cholangiocarcinoma, select the7postoperativehilar cholangiocarcinomaprognosis to producethe effects of non-repeat characteristic clinical factors analyzed by univariate analysis, multivariate analysis was performed by Cox regression model, analyzing impact factors of postoperative survival.Results:55cases the type of surgical resection, R0resection in38cases, R1resection in13cases, R2resection in four cases of perioperative death of twocases. Univariate analysis showed that in patients with hilar cholangiocarcinoma postoperative survivalfactors for tumor stage (χ~2=15.706,P=0.001<0.05),the degree of cell differentiation(χ~2=12.178,P=0.002<0.05)whether R0resection (χ~2=22.981,P<0.001), lymph nodes with or without metastasis (χ~2=9.349,P=0.002<0.05),vascular whether theviolation of (χ~2=4.571,P=0.033<0.05),surgical way (χ~2=10.854,P=0.001<0.05);but Multivariate analysis showed that, whether R0resection and lymph node metastasis (P<0.05) affect an independent risk factor of postoperative survival of patients with hilar cholangiocarcinoma.Conclusion:R0resection (negative margins) and lymph node metastasis is the impactof hilar cholangiocarcinoma patients’ survival of the main determinants.
