Abstract/Summary:
Objective To evaluate the efficiency of Fluency stent-grafts intransjugular intrahepatic portosystemic stent shunt (TIPS).Methods48cirrhosis patients with esophagogastric variceal bleeding(EGVB) treated by TIPS using Fluency stent-grafts. Portal venouspressure,liver function(including albumin, total bilirubin andprothrombin time) were compared before and after operation. The48caseswere strictly followed for1year. To evaluate the clinical effect,Therecurrent bleeding rate, The relapse rate of shunt occlusion,incidenceof hepatic encephalopathy、the incidence rate of hepatic failure andmortality were recorded and analyzed in one year.Finally,analyzing thefactor of stenosis.Results TIPS creation was successfully accomplished in all48patients,Forty stent-grafts were successfully implanted.42stent grafts were8mm,4were7mm,4were6mm in diameter. The covered length of the stents variedfrom4to8cm.Mean portal venous pressure decreased from preoperation(36.83.5cmH2O) to postoperation (26.73.5cmH2O)(P<0.05). Nosignificant differences at the level of albumin,total bilirubin and prothrombin before and after operation (P>0.05). The48cases werestrictly followed up for1year,The relapse rate of shunt occlusion was14.58%(7/48),six cases were hepatic venous stenosis,one case was portalside stenosis,The relapse rate of re-bleeding was12.50%(6/48), Theincidence rate of hepatic encephalopathy was20.83%(10/48), the incidencerate of hepatic failure was2.08%(1/48),the mortality was8.33%.Conclusion Fluency stent-graft is safe and effective in TIPScrention,with high patency and low re-bleeding rates. Most of the stenosisis at the end of the stents,The stent must be long enough.The end of thestent must extend the inferior vene cava and portal vein.
