Effects Of Candesartan On Structural Remodeling And Electrical Remodeling Of Left Heart In Hypertension Patients With Paroxysmal Atrial Fibrillation

Objective To observe the effect of candesartan on structural remodeling and electrical remodeling of left heart in hypertension patients with paroxysmal atrial fibrillation.Methods One hundred hypertension patients with paroxysmal atrial fibrillation were enrolled and divided randomly into the treatment group(n=50) and control group(n=50). The patients in treatment group took candesartan4mg per day arally and the patients in control group took felodipine5mg per day orally. The course of the treatment was12months. In period of treatment other ACEI and ARB drugs were forbidden to take to all patients. We did holter and echocardiogram before and after12-months drug therapy, and did ECG regularly.Results After12months treatment, there were93patients who have completed the whole study. There were46patients in treatment group and47patients in control group. The systolic and diastolic pressure of the two groups decreased significantly (P<0.05), but there were no obvious difference between the two groups(P>0.05). The LADd and LVMI of treatment group were decreased obviously compared with control group (P<0.05). The Pmax and Pd of two groups were decreased compared with before, and the declines of treatment group were more obvious compared with control group(P<0.05).The recurrence rate of atrial fibrillation of treatment group was significant decreased,and the rates of maintain sinus were also higher (P<0.05).Conclusion Candesartan can decrease Pd, improve left atrial function and reverse left ventricular hypertrophy while can reduce the blood pressure in hypertension patients with paroxysmal atrial fibrillation, then can reduce the occurrence and recurrence rate of atrial fibrillttion.
