Objective To understand pathogen distribution and drug resistance of urinary system infection after percutaneous nephrolithotomy (PNL), then help the clinic to use antibiotics rationally after PNL.Methods Clinical data of535patients who underwent PNL between September,2006and December,2011in our department were analyzed retrospectively. According to certain standard,461patients were selected as objects of study,Both blood and urine samples were cultured for those patients whose body temperatures were over38℃after PNL,only urine samples were cultured for those patients whose body temperatures were between37.3℃and38℃lasting beyond3days, strain identification and microbial sentitivity tests were further performed to those culture positive samples. Results There were64patients whose body temperatures were over38℃after PNL,18types and78strains of pathogen were cultured,pathogens presented in22(34.38%) blood samples and56(87.50%) urine samples. The blood culture showed that Escherichia coli,pseudomonas aeruginosa,Klebsiella and Staphylococcus were the predominant bacteria, accounted for31.82%(7/22),22.73%(5/22),13.64%(3/22),31.82%(7/22), respectively. The urine culture showed that the above bacterials were also the predominant bacteria, accounted for28.57%(16/56),21.43%(12/56),7.14%(4/56),19.64%(11/56), respectively. There were51patients whose body temperatures were between37.3℃and38℃lasting beyond3days.13types and29strains of pathogen were cultured,pathogens presented in29(56.86%) arine samples. The urine culture showed that the above bacterials accounted for20.69%(6/29),24.14%(7/29),6.9%(2/29),27.59%(8/29), respectively.Microbial sentitivity tests demonstrated that Gram negative bacilli were sensitive to imipenem、meropenem、panipenem/betamipron、cefoperazone and sulbactam/piperacillin sodium.Gram positive coccus were sensitive to tigecycline、 linezolid、nitrofurantoin、teicoplanin、compound sulfamethoxazole and vancomycinConclusions The outcome of the culture was similar:Bacterial’s distribution are multiple, and the majority pathogen is Gram negative bacteria, Escherichia coli and pseudomonas aeruginosa were the predominant bacteria,staphylococcus was the main type of Gram positive bacteria.A big variant resistance is found among different bacterium.
