Outcome Of IVF-ET Treatment In Polycystic Ovary Syndrome And Only Polycystic Ovarian Infertility Patients Analysis

Objective:By comparison with PCOS infertility patients,only ovarian ultrasonography polycystin changes in infertile patients and normal ovarian morphlolgy patients with IVF-ET in the clinical features and treatment outcome analysis,to assess PCOS and PCO infertility the outcome of treatment in IVF-ET differences and commonalities.Provide reference to the clinical diagnosis and treatment of PCOS and PCO infertile patients undergoing IVF-ET.Methods:Retrospective analysis from January2009to May2011in the reproductive center of First Affiliated Hospital of Guangxi Medical University carried out IVF-ET,divided into three groups:(1) According to ESHRE/ASRM diagnostic criteria77patients diagnosed with PCOS in PCOS group,a total of77fresh of IVF-ET cycles.(2) Only ultrasound showed ovaries were cystic changes,without blood biochemical performance,regular ovulation but combined with other factors of infertility,a total of162cases of fresh IVF cycle for the PCO group.(3) Random sample of in the period normal ovarian morphology,regular ovulation,to rule out ovarian response to gonadotropin response to adverse because of pure tubal factor infertility patients,a total of173cases of IVF fresh cycle of the normal control group.Three groups were compared between patients characteristics(age,infertility period,the basis of FSH,LH and LH/FSH,the vasis of the mumber of antral follicles),IVF treatment characteristics (Gn start dose, Gn days,the total Gn ampoules of HCG day LH,E2,P value)and IVF treatment outcomes(by the mumber of oocytes retieved,fertilization rate,cleavage rate,high-quality embryo rate,mumver of embryos transferred,cancel the transplant rate,clinical pregnancy rate of OHSS incidence,abortion rate).Results:1.Each group the average age,infertility period,the basis of E2,the basis of FSH,the proportion of primary infertility differenc not statistically significant(P>0.05), the PCOS group BMI,the basis of LH,LH/FSH,T compared with only PCO group and the control group increased, the difference was statistically significant (P<0.05). Only PCO group BMI,the basis of LH,LH/FSH,T compared with control group the proportion of primary infertility differenc not statistically significant(P>0.05)2.Three group of patients the day of HCG LH,P levels and the day of HCG endometrial thickness differenc not statistically significant(P>0.05);PCOS group and PCO group Gn start dose, the total Gn,average daily the amount of Gn than control group reducing,while the Gn number of days, the day of HCG E2levels increased, the difference was statistically significant (P<0.05). PCOS group Gn start dose, the total Gn,average daily the amount of Gn compared with PCO group differenc not statistically significant(P>0.05)3. Three group of patients cleavage rate,biochemical pregnancy rate,clinical pregnancy rate difference not statistically significant(P>0.05); PCOS group and PCO group the average number of oocytes,the number of fertilization, cleavage number,cancel the transplant rate,rate of OHSS incidence than the normal control group difference was statistically significant (P<0.05);PCOS group fertilization rate is lower than PCO group and normal control group,the difference was statistically significant (P<0.05);PCO group quality embryosmore is higher than PCOS group and normal control group, the difference was statistically significant (P<0.05);PCOS group spontaneous abortion rate is higher than PCO group and normal control group, the difference was statistically significant (P<0.05)Conclusion:PCOS patients the serumLH,LH/FSH,T and BMI increased,suggesting that endocrine disorders and obesity may also produce a variety of clinical manifestations of the reasons;isolated polycystic ovaries patients with endocrine and clinical manifestations similar to normal ovarian infertility patients. PCOS and isolated PCO patients responded to Gn insensitively,with less dosage of Gn use the number of days increase and more oocytes retrieval,OHSS risk and cancel transplant rate increased.In the process of IVF-ET treatment, PCOS have lower fertilization rate than isolated PCO,but PCOS have the same pregnancy chance;increased abortion rate of women with PCOS may be associated with lower auality of eggs and embryos,the potential of embryos continue to develop is affected.In the IVF-ET treatment, the clinical outcomes in isolated PCO patients are similar with the normal ovarian infertility patients.
