Objective To study the clinical value of64-slice spiral CT triple-phase enhanced scanning in the preoperative TNM staging of gastric cancer, in order to provide evidence for preoperative,reasonable, and personalized treatment strategies for gastric cancer.Methods Forty five patients diagnosed with gastric cancer were examined by64-slice spiral CT before operation between October2010to August2011in the department of gastrointestinal surgery and department of general surgery of Guangxi People’s Hospital, preoperative TNM staging was evaluated, which compared with intraoperative palpation and postoperative pathological results.Results Compared with intraoperative palpation and postoperative pathological results, the accuracy rate of forty five patients with gastric cancer preoperative T staging was84.44%(38/45), T1staging was100%(3/3), T2-3staging was84%(21/25), T4a staging was83.33%(10/12), and T4b staging was80%(4/5) by64-slice spiral CT triple-phase enhanced scanning.38patients with gastric cancer was performed D2lymphadenectomy, the accuracy rate of preoperative evaluating the gastric cancer perigastric lymph node metastasis whether or not was92.1%(35/38); the accuracy rates of preoperative evaluating gastric cancer N0~N3b staging and N staging were71.4%(5/7),62.5%(5/8),81.82%(9/11),75%(6/8),25%(1/4) and68.42%(26/38), respectively. The accuracy rate of preoperative evaluating the gastric cancer distant metastasis whether or not was93.33%.Three patients with gastric cancer were found the peritoneal metastasis in intraoperative exploration, but were not found on the CT image.Conclusions The accuracy rate was high in evaluating the depth of infiltration in gastric cancer by64-slice spiral CT triple-phase enhanced scanning, which can provide valuable information in determining resectability of the tumor. The use of64-slice spiral CT triple-phase enhanced scanning can make more reliable judgment whether perigastric lymph node metastases, which can provide reference for reasonable and personalized treatment strategies for gastric cancer. The sensitivity of evaluating peritoneal metastasis in gastric cancer was low by64-slice spiral CT triple-phase enhanced scanning.
