Objactive:Observed the efficacy of cervical double-door laminoplasty in the treatment of cervical spondylotic myelopathy, and to explore the influencing factors of the treatmentMethods:Follow-up32cases of patients with cervical spondylotic myelopathy in the Second Hospital of Jilin University between2009and2012. The surgical approach is posterior cervical double-door laminoplasty. Follow-up time ranging from3to36months. The average age of the patient during surgery was48.7years. Observed indicators:1. Preoperative and postoperative6months,12months,24months JOA score and recovery rate.2. Measurement the preoperative and postoperative sagittal diameter by CT.3. Determination of preoperative and postoperative spinal cord flat rate.Results:JOA score increased from7.2to12.9±2.7、13.4±2.5and13.2±2.8at6、12and18months (P<0.001); The increase of sagittal diameter was5.3mm (P<0.001) Spinal cord flat rate from41.3±2.3%to50.2±4.6%Conclusion:Posterior cervical double-door can increase the bony canal volume, restore the blood supply of spinal cord.Posterior cervical double-door laminoplasty is an effective、simple and safe surgical treatment of cervical spondylotic myelopathy。But the efficacy was affected by the course of the disease, timing of surgery, intramedullary abnormal signal and other factors. |
