Objective:to analyze the biological behavior and category of ovarian clear cell carcinomaMethods:Totally25patients with ovarian clear cell carcinoma and116patients with ovarian serous carcinoma were analyzed retrospectively, who were diagnosed and treated in Women’s Hospital, School of Medicine, Zhejiang University during January2002and December2008. Ovarian serous carcinoma were subdivided into low grade and high grade types. The clinical-pathological characteristics, response to chemotherapy, and prognosis were compared.Results:the mean age of patients with ovarian clear cell carcinoma was55year. The percentage of early stage in ovarian clear cell carcinoma patients was84%, which was significant higher than that in high grade and low grade serous carcinoma (P<0.001and P<0.001, respectively). The ratio of residual tumor size<2cm in ovarian clear cell carcinoma was96%,which was significant higher than that in high grade serous carcinoma (P=0.021)and similar to low grade serous carcinoma(P=0.383).Recurrences occurred in28%) ovarian clear cell carcinoma and47.5%high grade serous carcinoma patients, the difference was not statistically significant (P=0.085).The3-year overall survival rate was81.1%in ovarian clear cell carcinoma patients, which was much higher than that high grade serous carcinoma patients(P=0.031) and was similar to that in low grade serous carcinoma patients (P=0.538). The3-year progression-free survival rate in ovarian clear cell carcinoma patients was61.7%, which was much higher than that in high grade serous carcinoma (P=0.042) and was similar to that in low grade serous carcinoma patients (P=0.770).Conclution:the linical-pathological characteristics of ovarian clear cell carcinoma were different from ovarian serous carcinoma. Compared to high grade serous carcinoma, ovarian clear cell carcinoma presented more early stage patients, more frequency of optimal cytoreductive surgery rate, lower recurrence, and higher3-year survival. It seems reasonable that ovarian clear cell carcinoma is classified into type I ovarian epithelial carcinoma.
