Background:African Americans (AAs) suffer from high burdens of heart failure. However, evidence on how heart failure disproportionately affects AAs is not clear.Objectives:To explore prognosis in patients with heart failure and associated risk factors between men and women in AAs.Methods:A random sample of122AA patients with heart failure aged26to90years older (M=67, F=83), who received clinical treatment at the Division of Cardiology of Hahnemann Hospital was studied. Differences in biomarkers (serum lipids, glucose, creatinine, HGB, ALB, white blood cell), left ventricular ejection fraction (LVEF) and LV mass were compared between males and females. The prognosis of heart failure was defined based on AHA/ACC stages. Logistic regression model and Cox hazard proportional regression models were applied to explore the relationship between biomarkers and prognosis, and between gender differences.Results:Serum total cholesterol, LDL, and CRE levels, and measures of LVEF, and LV mass were significantly different between men and women (P<0.01). Females with heart failure had a lower risk to develop advanced AHA/ACC stages C and D compared to males (OR:0.23595%CI:0.058,0.95). After adjustment of biomarkers, BMI, age, female seems still had a lower risk to develop AHA/ACC stage C&D heart failure, however, this association was not significant (OR:0.27,95%CI:0.04,2.06). LV mass as a risk factor for heart failure, patients with higher LV mass might be more likely to develop AHA/ACC stage C&D heart failure in male (OR:1.021,95%CI:1.006,1.037).Conclusions:There were significant differences in cholesterol, LDL and CRE, LV mass and LVEF by gender among AAs. The gender differences in the prognosis the heart failure in terms of AHA/ACC stages were also exist in AAs.
