Objective: Investigate the expression and significance of Cyclin D1, K17, CD34,Cathepsin S and Cathepsin D in the lesions of scalp of psoriasis vulgaris and scalpseborrheic dermatitis patients.Methods:(1) Lesions came from19patients with psoriasis vulgaris,16cases ofseborrheic dermatitis,14cases of normal and11cases of patients with eczema;(2)Conventional paraffin producer, HE dyeing, and immunohistochemistry methods detectionthe expressions of Cyclin D1, K17, CD34, Cathepsin S and Cathepsin D in the epidermisand scalp appendages.Result:(1)HE staining, psoriasis vulgaris scalp lesions mainly manifested as diffusehyperkeratosis, parakeratosis, granular layer decreased or disappeared, acanthosis andregular elongation of epidermal rete ridges, partially visible Munro micro abscess and/orKogoj spongiform pustules, dermal papilla extend, nipple microvascular increased withtortuosity and dilatation, leather shallow and perivascular mononuclear cell infiltration andsebaceous gland atrophy, infundibular following around the hair follicles withoutinflammatory cell infiltration; seborrheic dermatitis lesions histopathological changes issimilar to that of psoriasis vulgaris, but with the lighter, the sebaceous glands alsoatrophy.(2) In the scalp psoriasis and seborrheic dermatitis, Cyclin D1is mainly expressedin the basal layer of the epidermis and the prickle cell layer of the lower, two group ofpositive strength without significant(χ~2=6.4751,P=0.0907), but were higher than those of normal controls(χ~2=21.4135,P<0.0001;χ~2=9.8578,P=0.0072).(3) K17in scalp psoriasis rashof epidermal spinous layer, stratum granulosum and corneum showed a strong positiveexpression of, is higher than the scalp seborrheic dermatitis (mainly in the spinous layer inthe upper part, the stratum granulosum and corneum)(χ~2=7.4025,P=0.0247),2of6casesof eczema the epidermis has a small amount of expression, the normal group is negative.(4)The MVD values in dermal papillary layer of scalp psoriasis and seborrheic dermatitisrash are increased, and the former is bigger than the latter(t=-11.90,P<0.0001), while thelatter is greater than the normal group and eczema group(t,=8.64,P<0.0001;t=-3.73,P=0.0013).(5) Cathepsin S in scalp psoriasis and seborrheic dermatitis expression mainly inthe basal layer, the prickle cell layer and granular layer, but the expression intensity ishigher than that of the latter(χ~2=15.4578,P=0.0004), normal scalp skin is negative; only3cases of eczema has a small amount of expression in the lesions epidermis, and is mainlylocated in the upper spinous cell layer and granular layer. Cathepsin S and Cyclin D1expression in Skin lesions of psoriasis vulgaris was correlated(rs=0.36437,P=0.0245).(6)Cathepsin D in scalp psoriasis and seborrheic dermatitis epidermis (basal cell layer,spinous cell layer, stratum granulosum and corneum) showed expression, normal group isweak expression (only found in the stratum granulosum and corneum) or negative, thepositive intensity of psoriasis vulgaris and seborrheic dermatitis without difference,(χ~2=5.3532,P=0.0688), while the latter is greater than that of the normal group(χ~2=22.4272,P<0.0001). Cathepsin D and Cathepsin S, Cyclin D1and K17expression in skin lesionsof psoriasis vulgaris were correlated(rs=0.32733,P=0.0449;rs=0.59888,P<0.0001;rs=0.27735,P=0.0918).(7) In the scalp adnexal, Cyclin D1express in the infundibular portion of thehair follicle, outer hair root sheath and eccrine duct section. Inner root sheath and eccrinesweat secretion is negative. Psoriasis vulgaris and seborrheic dermatitis group expressionwas higher than that in normal group (χ~2=21.4135,P<0.0001;χ~2=15.6293,P=0.0004;χ~2=12.0180,P=0.0025)(χ~2=9.8578,P=0.0072;χ~2=13.7531,P=0.0010;χ~2=12.0519,P=0.0024),eccrine duct psoriasis is higher than seborrheic dermatitis(χ~2=4.7719,P=0.0289), other siteshave no significant difference; psoriasis vulgaris and seborrheic dermatitis rash the infundibular portion of the hair follicle, inner and outer hair root sheath K17positive,eccrine is negative, the infundibular portion of the hair follicle of positive degree theformer is higher than that of the latter(χ~2=9.5534,P=0.0084), the normal group theinfundibular portion of the hair follicle and eccrine is negative, inner and outer hair rootsheath groups had no significant difference, but the outer hair root sheath group is higherthan that in the normal group(χ~2=22.4722,P<0.0001;χ~2=18.2445,P=0.0001); hair follicle lowersection of the outer hair root sheath in psoriasis vulgaris, seborrheic dermatitis andcontrols groups showed CD34expression, while that of the infundibular portion of the hairfollicle, inner hair root sheath, eccrine is negative; Cathepsin S and Cathepsin D express inthe infundibular portion of the hair follicle, inner and outer hair root sheath and eccrineductal and secretion, Cathepsin S in the infundibular portion of the hair follicle of thepsoriasis vulgaris group was higher than that of seborrheic dermatitis group(χ~2=15.4578,P=0.0004), the latter is more than normal group (χ~2=29.0000,P<0.0001), That of the otherparts had no significant difference, but are higher than those in normal group (inner hairroot sheath with the exception)(χ~2=9.2391,P=0.0024;χ~2=15.6533,P=0.0004;χ~2=12.0015,P=0.0025)(χ~2=4.1429,P=0.0418;χ~2=16.4519,P=0.0003;χ~2=10.8069,P=0.0045); Cathepsin Din eccrine duct the psoriasis vulgaris group is higher than that of the seborrheic dermatitisgroup(χ~2=6.0395,P=0.0140), That of two groups of other parts have no significantdifference, but are higher than that in normal group(χ~2=18.9298,P<0.0001;χ~2=18.3888,P=0.0001;χ~2=12.5285,P=0.0004;χ~2=23.1779,P<0.0001;χ~2=21.7544,P<0.0001);(χ~2=17.1633,P=0.0002;χ~2=14.5000,P=0.0007;χ~2=6.2513,P=0.0124;χ~2=16.0536,P=0.0003;χ~2=14.6593,P=0.0007).Conclusion:(1) Histopathological changes in the scalp lesions of seborrheicdermatitis and psoriasis vulgaris is very similar, and both have sebaceous gland atrophy,excessive proliferation of epidermal cells,the expression of K17and dermal papilla layerMVD values increase, which are characteristic changes in the psoriasis vulgaris. Therebywe think of seborrheic dermatitis is likely to be a light or incomplete type psoriasis.(2)Cathepsin S and Cathepsin D in the epidermal cells of scalp lesions of psoriasis vulgaris and seborrheic dermatitis showed the same positive expression patterns, which relate toCyclin D1and K17, and express of Cathepsin S and Cathepsin D in the normal controlsgroup were negative or weak. Those show the expression of Cathepsin S and Cathepsin Drelate to the genesis and development of psoriasis vulgaris and seborrheic dermatitis,especially the excessive proliferation of epidermal cells and abnormal differentiation.(3)Cyclin D1K17, Cathepsin S and Cathepsin D in the infundibular portion of the hair follicleand outer hair root sheath of scalp lesions of psoriasis vulgaris and seborrheic dermatitisis expresed and level of the Cathepsin S and Cathepsin D in eccrine ductal and secretionwas higher than that of normal control group, show in the scalp lesions of psoriasisvulgaris and seborrheic dermatitis, the epithelial cells of follicular infundibulum and outerhair root sheath and eccrine duct section and secretion also produce pathologic changes.
