Retrospective Analysis On Perioperative Risk Of Fracture O F The Femoral Neck In The Elderly By POSSUM Scoring System

ObjectiveSummarize and analyze peri operative medical records of femoral neck fractures in the elderly, use physiologic and operative severity score system (POSSUM) to assesss risk of urgery in elderly patients with femoral neck fracture, and explore peri-operative treatment methods for the femoral neck fractures in the elderly survive.By analyzing the differences between the observed values and forecast values, assessed the application value of rates of postoperative complications of the scoring predicted group.MethodsReview analysis of patients records informationin, who age above65and accept surgery of oldfemoral neck fracture in the Trauma Orthopedic Department Guangdong Province Hospital in the period from October2008to October2010. Summarized and analyze internal complication before operation, operation in the processing, complications situation after operation. Following POSSUM scoring system and Copeland complications occurs rate calculation formula, compare actual complications onset number and forecast complications onset number.ResultsA total of142cases this group, men48cases, female94cases, average age77.37years old, average Physiology Score17.3and operative severity score13.97on average. GARDEN Ⅰ-3cases,2.11%;13cases of type Ⅱ,9.15%, Ⅲ-53cases,37.32%,73cases of type Ⅳ,51.41%.92patients with preoperative diagnosis with an obvious medical diseases, complications of44cases, complication rate is31.69%. POSSUM scoring system predicting number of postoperative complications is41cases, actual number44cases of complications, no significant difference in risk level size comparison of linear block (P>0.05).ConclusionOlder femoral neck fracture patients in peri-operational period when there is a risk, but through the reasonable prevention and treatment can ride out around the time of surgery. Preoperative coexist disease risk factors for postoperative complications, and its relevance. Deal with preoperative medical coexist disease, on peri operative security is very important. Age, sex, the operat ion time, the operation method, perioperative blood transfusion, operation time, preoperative and postoperative use traditional Chinese medicine of complications of direct link does not exist. POSSUM scoring system in old femoral neck fracture patients who underwent the operation risk forecast good practical value.
