BackgroundBrain demage in preterm infants mainly cause spasticity, athetosis and mixed thee types of cerebral palsy. Infants with low birth weight and perinatal hypoxia predisposed to spastic cerebral palsy; and neonatal asphyxia and bilirubin encephalopathy predispose dyskinetic cerebral palsy; hemorrhagic brain injury in children, such as intraventricular or peri ventricular hemorrhage susceptible to mixed-type cerebral palsy. While the brain injury in preterm infants is not only one of the major causes of cerebral palsy, also cause mental retardation, tic disorder, attention deficit disorder syndrome, sensory integration disorder, motor retardation, language developmental delay and other sequelae of central nervous system, all these can bing a great deal of burden on family and society. These sequelae in infancy are very difficult to find or be pay attention for the parents, and most of the sequelae were obvious found in peschool and school-age, so often missed the best treatment time and the probability of recovery is also greatly reduced. Therefore, how to avoid or reduce the happening of the sequela, how to reduce the degree of disability are become one of the problems demanding prompt solution.Object iveA randomized controlled trial was conducted to objectively assess the efficacy of the Massage of Tongduxingnao and Yishenjianpi and the Massage of Sensory Stimulation on neural development in the brain damage in preterm infants. The effects of the two groups is compared to see whether there are superiority in early intervention of brain damage in preterm infants with the Massage of Tongduxingnao and Yishenjianpi. Whether to further established the important position of traditional Chinese medicine in the early intervention of brain damage in premature infants and further to integrate between traditional Chinese medicine and Western medicine in early intervention.MethodsA randomized controlled trial was conducted.82infants with Brain demage in preterm infants in Nanhai Affiliated Maternity and Children’s Hospital of Guangzhou University of Chinese Medicine were selected, randomly divided into two groups. The experimental group received the treatment of Massage of Tongduxingnao and Yishenjianpi combined with routine intervention. The control group received Massage of Sensory Stimulation combined with routine intervention. The massage treatment of each time was lasted30-40minutes,1time a day,20days as a treatment course,3treatment courses would be taken(total3months). The Gross Motor Function Measure, Bayley scales of infant development Ⅱ (MDI、PDI) and the Gesell Developmental Scales were tested before, right after and3months after the treatment designed. the brainstem auditory evoked potential baep(BAEP), Visual evoked potential(VEP), electroencephalogram(EEG), Transcranial doppler color dopplar ultrasound(TCD) and Head imaging examination were tested before, and some items were tested again after the treatment designed which resuslt of the above was abnormal before,All data would be analyzed by SPSS13.0.Results1. The children’s development quotients (DQ) in both groups are improved. The experimental group has a significant superiority in improving the DQ of gross motor function by Gesell Developmental Scales compared with the control group(P<0.05), but no discrepancy in the social adaptation,social intercourse tested,fine movement and linguistic DQ by Gesell Developmental Scales compared with the control group (P>0.05).2. The scores of A and B areas (GMFM) are higher when tested right after the treatment (P<0.05). After the treatment, the score of A area in the experimental group is higher than the control group (P>0.05). The scores of B areas in the experimental group are higher than the control group3months after the treatment (P<0.05)3. After the treatment, the score of experimental group reach normalization in Mental Development Index(MDI) and Psychomotor Development Index(PDI) by BSID is higher compared with the control group(P>0.05).4. There are some recovery both groups on BAEP, VEP, TCD and EEG after treatment. Compared the two groups, but no discrepancy on these4examinations (P>0.05).ConclusionThe Massage of Tongduxingnao and Yishenjianpi and the Massage of Sensory Stimulation can promote the developmental level of the intelligence and gross motor function, can promote the recovery of visual and auditory,also can promote the recovery of brain blood supply, and The Massage of Tongduxingnao and Yishenjianpi has a significant superiority in improving the DQ of gross motor function and sitting position (B area of GMFM).
