Objective:To investigate the causes and the clinical feature of adult patients with fever of unknown origin (FUO), in order to improve the level of diagnosis, reduce FUO causes of misdiagnosis happen.Methods:The clinical data of107cases of adult patients with FUO in the department of infectious diseases of the First Affiliated Hospital of Guangxi Medical University from January2008to December2010were retrospectively study.Results:107cases had final diagnosis by various screening or diagnosis treatment. There were five causes in the development of FUO diseases, including infectious diseases, autoimmune conditions, malignancies, miscellaneous and undetermined origin accounted for64.5%(69/107),11.2%(12/107),7.5%(8/107),5.6%(6/107) and11.2%(12/107) respectively. Infectious diseases and autoimmune conditions were more frequently happened in the patients below the age of40years. Infectious diseases and malignancies were more frequently happened in the patients over the age of40years. Serology and bacteriology led to final diagnosis in32.6%(31/95). Physical examination and analysis, imaging examination, biopsy, inspection contributed to the diagnosis in29.5%,21.1%,8.4%and8.4%respectively. The main pathogens responsible for the infectious diseases were Typhoid and paratyphoid fever (23.2%,16/69). Adults Still disease was the main forms of autoimmune condition (33.3%,4/12). Lymphoma was the main forms of malignancy (50.0%,4/8).Conclusions:Infections was the most primary cause for patients with fever of unknown origin at ED. The main pathogens responsible for the infections was Typhoid/paratyphoid. These patients had their special clinical features. Their prognoses were all different.
