Diagnosis And Management Of13Case Of Cesarean Scar Pregnancy

Objective:To study the early diagnosis and treatment of cesarean scar pregnancy(CSP).Methods:A retrospective analysis of during a10-year period from Dec.2001to Dec.201113patients were diagnosed as CSP and treated conservatively to preserve fertility in The Second Clinical Hospital,Shanxi Medical University. Reorganization and the analysis the clinical data, medical history, symptoms, signs, auxiliary examination, diagnosis and treatment characteristics.Results:In13patients were diagnosed after cesarean uterine scar pregnancy, the operation methods of uterine lower segment transverse incision. The average age of the13patients was33.5years(23～41), there are1cesarean section pregnancy history, the intervalrom the last cesarean was7months to9years, The average gestational age of76.2d(41～134d). Among the13women,12/13(12cases) complained of vaginal bleeding and2/13(2cases) experienced abdominal pain. The13patients were admitted by color Doppler flow imaging diagnosis, laboratory blood B-human chorionic gonadotrophin increased. Among the13women,84.6%(11cases) were misdiagnosis give other treatment in the basic level hospital. The methods of treatment in our hospital included systemic or local methotrexate administration, uterine artery embolization, interventional chemotherapy, hysteroscopy and lesion excision, laparotomy and curettage. Fertility was preserved for all patients. The average period of hospitalization was16.8d(5～30d).Conclusion:The history of cesarean section and ultrasound examination for cesarean scar pregnancy diagnosis of CSP, CSP in newly diagnosed easily misdiagnosed in the basic level hospital. Suction curettage or interventional therapy after MTX conservation treatment therapy treatment of CSP is effective method, which can effective reduce the bleeding volume in uterine curettage.
