Objective:To investigate expression levels of XIAP and survivin in pleural effusion and bronchoalveolar lavage fluid(BLAF) and the diagnostic vualue for lung cacer.Methods:The pleural effusion and bronchoalveolar lavage fluid specimens were collected from60patients with malignant pleural effusion which were confirmed by cytology or histopathology and60patients with tuberculouspleurisy which were confirmed by nodular tuberculosis or acid-fast bacillus in sputum. Enzyme linked immunosorbent assay was used to detect XIAP and survivin of pleural effusion and bronchoalveolar lavage fluid.the rusults were analyzed with statisticalm ethods,estimating the clinical diagnostic value of two index.Results:1. XIAP levels in malignant pleural effusion were (84.83pg/ml±27.40pg/ml) higher than in tuberculosis pleural effusion (47.23pg/ml±21.58pg/ml)(P<0.05);XIAP levels in BALF (85.02pg/ml±42.57pg/ml) were higher in the patients with malignant pleural effusion than (43.45pg/ml±29.32pg/ml)(P<0.05)in the patients with tuberculosis pleural effusion; XIAP levels were not different between pleural effusion and BALF in lung canscer group (P>0.05)。2. survivin levels in malignant pleural effusion were (85.54pg/ml±29.18pg/ml) higher than in tuberculosis pleural effusion (42.49pg/ml±20.07pg/ml)(P<0.05);XIAP levels in BALF (95.27pg/ml±34.54pg/ml) were higher in the patients with malignant pleural effusion than (44.99pg/ml±20.09pg/ml)(P<0.05) in the patients with tuberculosis pleural effusion;The levels of XIAP and survivin in lung cancer were significantly higher than those of benign lung diseases (P<0.05); Survivin levels were different between pleural effusion and BALF in lung canscer group (P<0.05).3.The sensitivity,specitivityand accuracy of detecting XIAP、survivin in pleural effusion were85%、90%;80%、85%;82.5%、85%; The sensitivity,specitivity and accuracy of combinative detecting XIAP、survivin in pleural effusion were95%、75%、85%。4. Thesensitivity,specitivity and accuracy of detecting XIAP、survivin in bronchoalveolar lavage fluid were75%、95%;80%、75%、77.5%、85%。The sensitivity,specitivity and accuracy of combinative detecting XIAP、survivin in bronchoalveolar lavage fluid were95%、70%、85%。Conclusion:1. The detection of XIAP、survivin has dianostic values in differentiating between malignant pleural effusion and tuberculous pleural effusion, but it has higher value to differentiate benign and malignant pleural effusion by combining them together.2.The detection of XIAP、survivin in bronchoalveolar lavage fluid has dianostic values for lung cancer,combining them can improve positive rate;3. The expression of XIA P between bronchoalveolar lavage fluid of lung cancer group and malignant pleural effusion was not significant (P>0.05); but the levels of Survivin between bronchoalveolar lavage fluid of lung cancer group and malignant pleural effusion was significant (P<0.05),it indicated that The detection of Survivin in bronchoalveolar lavage fluid has greater significance for diagnosis of lung cancer。...
