| BackgroundsRadiotherapy is a commonly used method of breast cancer comprehensive treatments. However, it can cause a series of local and systemic adverse reactions, which seriously influence the quality of life. The period of radiotherapy is defined as periods from the beginning to one week after radiotherapy [1]. The advantages of Chinese medicine treatment can improve immune function and reduce the radiotherapy toxicity, therefore, it has broad indications and unique advantages in breast cancer comprehensive treatments [2]. However, the study of traditional Chinese medicine in breast cancer radiotherapy period is still staying on the treatment of side effects disease, unable to highlight the features and advantages of TCM on overall concepts and dialectical differentiation. Even some experts suggest it should be treated with dialectical differentiation, however, the name of the syndromes lack of unified standard, therefore, it obstructs the further research of TCM for the period of radiotherapy to some extent. Especially it becomes the main bottle-neck of further clinical development that the classification of the syndrome is not standard.Object ivesThis study is to explore traditional Chinese medicine syndrome classification and the symptoms of each type in the period of radiotheraphy, in order to form TCM.specification first draft and provide a theoretical basis for next diagnosis and treatment.MethodsWe formulate "syndrome information collection form of the period of radiotheraphy " and "the clinical syndrome information collection practices (SOPs) of the period of radiotheraphy", employ cross-sectional observation, collect TCM clinical informations, establish a database, use case clustering analysis combining with expert discussion, determine the satisfaction of TCM syndrome classification, then fix and name each type, and confirm the primiary and secondary symptoms of each type by frequency analysis.Results1.240cases of breast cancer patients in the period of radiotheraphy are clustered of three, four, five analysis respectively. Through statistics analysis and experts discussion, the results show that the four types conform to clinical practices, so we identify four types in practice.2. Then we revise and name each type:deficiency of both qi and yin type, fire flaming type, deficiency of both spleen and kidney type, and deficiency of both qi and blood type. Furthermore, we confirm the the primiary and secondary symptoms of each type by frequency analysis.Conclusions1.Clustering four types is compatible in radiotherapy period of breast cancer:deficiency of both qi and yin type, fire flaming type, deficiency of both spleen and kidney type, and deficiency of both qi and blood type.2. Through frequency analysis of the four clustering, the primary and secondary symptoms of each type are as follows:(1)deficiency of both qi and yin type:primary symptoms:mouth parched and tongue scorched, shortness of breath, fatigue and disinclination to talk, body tired fatigue, menstrual disorders or amenorrhoea, emaciation, insomnia or forgetfulness, dark urine, flashed face, and red tongue, white thin moss, thready pulse, rapid pulse; secondary symptoms:palpitation and terrified, stool stem node, burning sensation of five centres, pink tongue;(2) fire flaming type:primary symptoms: mouth parched and tongue scorched, skin redness, swelling, heat and pain, pruritus unable to endure, oral ulcer, dry skin taking off bits, gingival bleeding with swelling and pain, dry cough with little phlegmy, sore throat, menstrual disorders or amenorrhoea and red tongue, thin yellow moss, thready pulse, rapid pulse; secondery symptoms:chest distress and dyspnea, phlegm with blood, taking food with burning sensation, dysphagia, pink tongue;(3) deficiency of both spleen and kidney type:primary symptoms:anorexia or satiety after taking food, soreness of waist, cold body and limb, dizziness, upper limb swelling, loose stool, tinnitus, and body fatigue, clear abundant urine, frequent urine at night, pale tongue, thin white moss weak, deep pulse, thready pulse; secondery symptoms:fatigue and disinclination to talk, menstrual disorders or amenorrhoea;(4) deficiency of both qi and blood type: primary symptoms:sweat out increasing with activity, menstrual disorder or amenorrhea, yellow or pale complexion, pale tongue, thin white moss, thready pulse; secondery symptoms:insomnia forgetful, dizzy, fatigue and disinclination to talk, body tired fatigue, palpitation and terrified, shortness of breath, pale tongue. |
