Objective:Through detailed clinical risk assessment of preoperative and postoperative patients, this paper recorded some risk indices associated with Deep Vein Thrombosis(DVT), analysed the correlation between risk factors and the formation of the postoperative DVT, investigated risk factors and the incidence of DVT after oral and maxillofacial surgery. All these would provide the basis for the prophylaxis of DVT.Methods:There were103selected patients who had accepted general anesthesia in Department of Oral and Maxillofacial Surgery of Provincial Hospital affiliated to Shandong University from June1,2011to Feb.1,2012. On the day of hospitalization, the author had finished the basic data collection, including age, gender, height, weight, recent surgery, and injury history, history of basic diseases, and so on. On the second day of hospitalization, we drew vein blood for testing correlated blood coagulation indices including D-dimmer, PT, APTT, TT, FIB and Selecting CDFI as the only standard for diagnosing DVT. At the same time, we checked CDFI to rule out patients with lower limbs DVT before surgery. If blood coagulation indices and CDFI results were normal, we proceed with surgery in suitable time. We examined correlated laboratory indexes, recorded BMI, operation time, bed time on the first day of post operation. According to Caprini clinical risk assessment table of DVT, the patients were divided into four groups:0≤low-risk≤2points,3points≤middle <5points,6points≤high-risk≤8points, a very high-risk group>8points.3to7days after the day operation, CDFI was reexamined. After patients left hospital, we must record patient’s hospital stays. The statistics software SPSS17.0was used to conduct statistical analysis. It calculated the incidence of DVT after oral and maxillofacial surgery, tested the correlation between various risk factors and oral surgery patients suffering from DVT through the logistic regression, thus selecting risk factors of significantly statistical difference; described all kinds of influencing factors in Caprini groups, and made correlation analysis between influencing factors and Caprini groups。Results:Among103patients enrolled, the incidence of DVT in the study group was6.8%, and that of PE was0.97%, seven patients with postoperative deep vein thrombosis were identified during the study period,one evolved into PE. Four cases occurred in the left lower limb, one cases occurred in the right lower limb, two cases occurred in the bilateral lower extremity. Logistics regression analysis was concluded that age, malignant tumor, D-dimmer and FIB had significant correlation with DVT,(P<0.05), According to the correlation analysis, there were significant statistical difference between age, BMI, operative time, bed time, hospitalization time, malignant tumors, D dimmer and FIB and Caprini groups.(P<0.01).Conclusion:The incidence of DVT in the field of Oral and Maxillofacial Surgery is very low. As the risk of grouping Caprini increasing, the risk of lower limbs DVT is greater and greater; advanced age and malignant tumor are predisposing factors for deep venous thrombosis. The numbers of the D-dimmers and FIB in the blood indicate the possibility of the formation of the DVT and have some of the early warning value.
