| Objective: To determine the range of total serum bilirubin(TSB)of healthy termnewborns during the first post-natal week in Huainan region of Anhui province andprovide the theoretical basis for the neonatal jaundice intervention in this region.Method:(1) The bilirubin analyzer for capillary blood is used to measure total serumbilirubin level of1257healthy term newborns during the first post-natal week, P25, P35,P75and P95are recorded at every time-point, the peak serum bilirubin(PSB) and thetime it appears is observed.(2) Pick up50blood samples at random and measure theirtotal serum bilirubin level with the routine laboratory method and at the same timemeasure their serum bilirubin level with bilirubin analyzer for capillary blood andtranscutaneous bilirubin (TcB) measurement, then calculate the linear correlationcoefficient of TcB level and TSB level, capillary blood bilirubin level and TSB levelrespectively to analyze their statistical significance, and get the linear regression modelat last.(3) Make a statistical analysis of the50newborns in terms of synchronous levelof TcB and capillary blood bilirubin level and make methodological comparision.(4)Record the newborns’ skin xanthochromia coverage scales and TSB levels in order toget the corresponding TSB level in terms of xanthochromia coverage.Result:(1)In this study,the serum bilirubin P25, P50, P75and P95of1257heal–thy term newborns are58.64μmol/L,72.51μmol/L L,102.44μmol/L,120.35μmol/L, re spectively in the first day,2th day105.41μmol/L,119.72μmol/L,150.18μmol/L,185.30μmol/L,3th day128.85μmol/L,157.95μmol/L,191.22μmol/L,227.61μmol/L,4thday160.24μmol/L,191.40μmol/L,216.65μmol/L,277.49μmol/L,5th day164.26μmol/L,179.51μmol/L,209.88μmol/L,263.74μmol/L,6th day131.87μmol/L,172.73μmol/L,195.57μmol/L,231.26μmol/L,and7th day118.94μmol/L,157.57μmol/L,204.83μmol/L,223.84μmol/L, with PSB294.46μmol/L appearing at the fourth day.(2) close correlation is existed between the capillary blood bilirubinmeasurement and the routine laboratory method, also the transcutaneous bilirubinmeasurement and the routine laboratory method.(3) Both capillary blood biliru-bin measurement and transcutaneous bilirubin measurement of serum bilirubinof healthyterm newborns show the serum bilirubin levels are significant in ter-ms of statistics in the first3days, the6th, and7th days (P<0.05) while theyare not in the4th and5th days (P>0.05).(4) The total serum bilirubin levelis112.3±6.2μmol/L when the xanthochromia is on the head,165.1±26.7μmol/L on the chest,214.0±30.8μmol/L on the belly,268.5±28.2μmol/L on the knee or elbow, and>271.7μmol/L on the palm or sole of the foot.Conclusion: The results of the study define the TSBvariation rang, PSB and its time ofhealthy term newborns within7days after their birth in Huainan region of Anhuiprovince, which can provide the theoretical basis for the neonatal jaundice interventionin this region; capillary blood bilirubin is reliable in measuring serum bilirubin level ofnewborns; visual assessment of xanthochromia coverage is also an easy and feasibleway of estimating serum bilirubin level initially. |
