| Object To study the distribution and characteristics of HLA-DRB1allele polymorphisms in acute onset autoimmune type1diabetes(Tl ADM) in China Hunan Hans.Design Cross-sectional and case control study.Methods208TIADM patients were measured for GADA,IA-2A by radioligand assay.A total of208T1ADM patients and226healthy controls were typed for genetic polymorphisms of HLA-DRB1with PCR sequencing-based typing method.The distribution frequency differences of HLA-DRB1alleles between T1ADM and controls in Hunan Hans were analysised, and the relationship between GADA,IA-2A and HLA-DRB1allele polymorphisms were studied.According to the age of onset more than15years or not,the patients with T1ADM were divided into juvenile-onset type1diabetes(JO) and adult-onset type1diabetes(AO),the distribution of HLA-DRB1allele polymorphisms in these two groups were analysised.Results①The positivity frequencies of GADA,IA-2A,both GADA and IA-2A were87.5%,35.1%,26.4%in T1ADM patients.②Compared with healthy controls,the allele frequencies of DRB1*0301and DRB1*0901were higher in T1ADM(P<0.01),and the frequencies of DRB1*1405was lower in T1ADM(P<0.01).③The positivity frequency of IA-2A were higher in TIADM patients carrying DRB1*0901(P<0.05),no significance difference was found in GADA,IA-2A positivity frequencies between TIADM patients carrying protective alleles or not(P>0.05).④The frequency of DRB1*0301allele were higher in JO compared with AO(P<0.01),no significance was observed in protective allele frequencies between JO and AO.Conclusions①The susceptibility alleles of HLA-DRB1in T1ADM in China Hunan Hans are DRB1*0301and DRB1*0901.And the protective alleles are DRB1*1405.②The patients with T1ADM carrying DRB1*0901have higher IA-2A positivity frequency.The results don’t show that protective alleles have an effect on GADA or IA-2A positivity frequencies.③The JO carry susceptible DRB1*0301alleles more frequently than AO. |
