Backgroud&Objactive:Atrial fibrillation(AF)is one of the most common arrhythmia with high morbidity and mortality. It can be divided into paroxysmal atrial fibrillation,persistent atrial fibrillation and permanent atrial fibdllation. until now,there are two kind of theories chiefly, including focal mechanism theory and single circuit re-entry theory.Treatment for atrial fibrillation call be divided into four categories:drug therapy, defibfiliation therapy, catheter interventional therapy and surgical treatment. Maze Procedure Using Radiofrequency Ablation is the most widely used technology in Surgical treatment. But at present,most hospital do this kind of operation using radiofrency ablation in the state of Arrested-Heart.However,for lack of effective evaluation method in maze procedure using radiofrequency ablation, the rate of freedom from AF is only70%, much lower than the rate of traditional maze procedure which is99%.So,how to improve the rate of freedom from AF becomes the main focus of research for the present. This study is a contrasting study of the maze procedure using radiofrequency ablation in patients with atrial fibrillation between Beating-heart group and arrested-heart groupMethods:Forty patients with atrial fibrillation and rheumaatic heart disease were divided into two groups:beating-heart group and arrested-heart group. Twenty patients in the beating-heart group underwent the Maze procedure by radiofrequency ablation in the state of beating-heart.The rest of them underwent this operation in the state of arrested-heart.Both of the two groups underwent the valvular heart surgery after undergoing the maze procedure by radiofrequency ablation.Results:All operation was successful, All patients in the beating-heart group were sinus rhythm after the operation. The rate of freedom from AF was100%. Four cases of them were recurrence of atrial fibrillation during the1to6days after the operation. But it restored sinus rhythm again without any Specific treatment in two weeks.14patients in the arrested-heart group were sinus rhythm after the operation.The rate of freedom from AF was70%. The rate of freedom from AF between the two groups has statistical difference(P<0.01).Conclusion:1. The effectiveness of Bipolar radio frequency ablation combined valve replacement for rheumatic heart disease combined atrial fibrillation is unambiguous, meaning of restoring sinus rhythm.2. Application of bipolar radiofrequency ablation in the treatment of AF in beating heart is superior to that of arrested heart. It is a safe and efficacy method which recommended to be applied widely.
