Objective:With the development of society and improved living standards.people’s concepts of health have gradually changed from the traditional medical mode going to hospital when sick to a prevention-oriented health care model. Changes in the model will inevitably bring new demand for medical services, as well as conversion of functions of all levels of medical service agencies in the health care services. Primary health institutions need to take on more responsibilities, including day-to-day health care knowledge, treatment of common diseases, regular medical checks for community residents, the establishment of residents’ health records, for the purpose of disease prevention and seeing the doctor in the community. However, due to the uneven distribution of medical resources in China, primary health institutions have very limited medical resources.and are not inadequate to play their role in the new medical model. For this problem.this paper presents a solution based on HL7engine for unimpeded exchange of medical information, for primary health institutions to get more guidance from experts, to improve their medical services.and perform their duties better.Methods:1. The overall system is classified into two parts:entry and view the medical records. apply the medical records."entry and view the records" is to manage the local medical data, including creating. modifying.researching the medical data "apply the medical records" establishes an unimpeded exchange path between primary health institutions and specialized hospitals to share the regional health resource.2. We achieve the function of "entry and view the medical records" by Visual Studio2005and SQL Server2008. to complete the preparation of the core code in Visual Studio2005. including the preparation of the user interface, formatting the medical data, operation and maintenance of medical records data. The medical record data is finally stored in SQL Server2008database. The database structure was optimized to improve the efficiency of access to the database.3. We achieve the function of "apply the medical records" by Visual Studio2005and HL7engine. On the basis of analyzing health information exchange international HL7 standard, we design and implement HL7engine using the technology of COM. which includes HL7message’s construction, parse, send and receive. The technology of XML and COM are used to achieve the function of HL7message’s construction and parse.and HL7message’s send and receive use TCP/IP protocol and C/S architecture. Any health institutions using this engine can quickly develop the system to send and receive HL7standard message, achieving the purpose of sharing health resource regionally and improving the utilization of finite resources.Results:Testing result shows, the electronic medical records system designed in the paper achieves the goal of maintaining two functions expected. Entry and update the records can be completed between20ms and30ms; View the medical records can be completed between220ms and290ms when the records of the database comes up to one million. The required time of apply the medical records is related to the rate of network transmission. The higher the rate of network transmission, the shorter the time required. When the rate of network transmission is2Mbps. apply the medical records can be completed between1s and2s.Conclusion:Compared with the community electronic medical record system exited, the system designed in the paper is more efficient in updating and viewing the medical records. Moreover.the interactive interface is more human. The HL7engine which is designed by COM can achieve the goal of unimpeded exchange for data in different formats in multiply health care system.providing a quick and easy way to increase the function of sharing the medical records for existing health care systems.
