Objection:To explore efficacy and safety of fine catheter decompression therapy in patients with chronic obstructive pulmonary disease (COPD) and different types of pneumothorax.Methods:One hundred and eleven COPD patients with pneumothorax were randomly divided into routine therapy group (n=57) and fine catheter decompression therapy group (n=55).The standard drainage tube was used in the routine therapy group. Based on the2times of evaluation results of gas partial pressure of pleural cavity, the type of pneumothorax can be accurately determined, and the effective exhaust measures were taken.Results:In total of112patients finished the clinical trail. Within24hours, the treatment were successfully between the large-bore and the small-bore catheter group respectively was10patients vs12.Within5d, the large-bore group has43patients with treated successfully,1with lost the drange tube,5with pneumoderm,2with blocked drange tube. The other group has28with treated successfully,5with lost the drange tube,19with pneumoderm,11with blocked drange tube. In the two groups, there were13patients blocked drange tube who undergoing evalue the gas partial pressure, after that there were6with closed pneumothorax treated by manual aspiration via placement of a small catheter,5with traffic pneumothorax and2with tension pneumothorax who both insertion of a large-bore catheter coupled to a water-seal device.Conclusion:1. The successful rate of treating COPD with pneumothorax between the large-bore and the small-bore catheter group, which was not sifnificance within24hours; but within5d the successful rate of large-bore more than the small-bore catheter group.2. The coincidence rate of pneumoderm and blocked drange tube in traffic and tension pneumothorax, which the small-bore catheter more than the large-bore catheter group.3. Pneumothorax with blocked drange tube or fail to treat, who based on the evaluation result of gas partial pressure of pleural cavity, and the effective exhaust measures were taken.
