Objective: To develop a device that to temporally close the abdominal defect. Toobserve the tolerance of infection and repair effect of a new composite patch and twobiological patches repairing abdominal wall defect in contaminated field in a dog model,to investigate their feasibility of one-stage repair in contaminated field. And to designnew patches can be used in contaminated field.Methods: First stage: Development of the device that can temporally close theabdominal defect: Refer to the construction of the hand-assisted laparoscopic cuff andthe air bladder of the air tube, we designed the device. It consists of two air bladders,and a pass-way on the centre. The bladders can stop bleeding when filled with gas.laparoscope equipments can be inserted into the abdominal through the centre pass-wayto diagnose and treat the abdominal trauma.Second stage: Twenty-four male dogs (weighing16-20kg) were enrolled in. three3×3cm abdominal defects were established at the upper, left and right lateral abdominalwall of one dog. The defects of each dog were repaired with three different patches (size5×5cm), there were three groups according to the patches used:polylactide-co-caprolactone (PLC) and collagen and polypropylene (PP) mesh wasgroup A, porcine cross-linked biologic patch was group B, and cattle cross-linkedbiologic patch was group C. Sprinkle1ml ileal juice on the defects to create acontaminated model. At90days postoperatively, the adhesions was observed, and thepatches and adjacent tissue was harvested for histological observation.Results: No death occurred after operation, most the incisions were well healed whilefew were flare around the incision. Only one in group A was abscess, after anti-infection treatment, the incision healed without taking out the patches. Each group occurredadhesion90days postoperatively. Histological study showed that the scores ofproliferating fibroblasts, inflammatory reaction and formation of new blood vesselswere higher in group A than in groups B and C.Conclusion:1The device can temporally close the abdominal defect, stop bleeding,stabilize the abdominal trauma. It could decrease the death due to blood lose during thetransfer. Meanwhile, it can use the laparoscope equipments to diagnose and treat theabdominal trauma.2biological meshes served as the supporting frame can welltolerance the infection, it is the first choice when used in contaminated field. But it’shigh price limit its wide use. Meanwhile, recurrence rate was higher than synthetic meshbecause lack of strength;3The effect of PP and collagen composite patch is similar asthe biological patch in contaminated field. it seems to have well tolerance of infection. Itmay could be used in contaminated field, though the proliferation of fibroblasts,inflammatory reaction and formation of new blood vessels was severs than biologicpatches.
