Background:Treatment of acute myocardial infarction (AMI) with stem cells as a promising method has made great progress, but still faces a series of challenges, Inflammatory response is one of the key factors to hinder the role of its efficacy after AMI.Objective:The present research was to evaluate adiposed-derived mesenchymal stem cells combined with valsartan for treatment of acute myocardial infarction and to explore its possible mechanism and provide experimental evidence for the clinical application.Methods and results:The first part:The adiposed-derived mesenchymal stem cells were isolated and cultured from both sides of the brown fat of rats scapula by enzyme digestion and adherent. Microscopy showed a large number of adherent cells, primary cells are mostly round or ova.mostly spindle-shaped, triangular after2-3passages.The cell phenotype at passages3were identified via flow cytometry and results showed that ADMSCs were positive for CD90and CD29, but negative for CD31,CD34and CD45.It demonstrated that we have successfully isolated and cultured ADMSCs from the fat of rats.The second part:32adult male SD rats were randomly divided into four groups (n=8):group A (control, simple AMI group), group B (ADMSC transplant),group C (valsartan group), group D (ADMSCs+valsartan group). The model was established by ligation of the left coronary descending artery in support of ventilator. About five minutes after ligation the left anterior descending artery ECG V1-V5, ECG ST-T was arched upward elevation and left ventricular anterior wall got white can be identified that model was successfully established. Selective four points in the border zone of AMI was implanted ADMSCs20×106/L in the Group B and group D (50ul of each point within1hour) with microinj ector and instantly given valsartan orally (30mg/kg/day, dissolved in5ml saline) treatment for4weeks in the group C and group D. During the model-making and the next one week, Four deaths due to ventricular fibrillation, two died of respiratory failure and excessive anesthesia respectively;the rest of the postoperative survival well. The results show that the ligation of left anterior descending artery to establish model of AMI is relatively stable and reliable. The third part: The expression of IL-1β, IL-6and TNF-α decreased in the blood of group C and group D decreased much significantly. Cardiac function and fibrosis in group D has been significant improvement, cardiomyocyte apoptosis in the combined group was significantly reduced in infarct borderConclusion:The adiposed-derived mesenchymal stem cells in combination with valsartan can improve the prognosis of myocardial infarction much significantly, and its mechanism may be related to survival, differentiation of transplanted stem cell and enhanced anti-apoptotic effects.
