Objective:To investigate the risk factors for the development of bronchopulmonary dysplasia and the severity in Low birth weight infants, so as to provide advice for the prevention and treatment of BPD.Methods:Data were collected retrospectively among the1932low birth weight infants admitted to Children’s Hospital of Chongqing Medical University between January1st2010and December31st.Among them,50cases who survived and used oxygen therapy over28days, defined as BPD group,9cases excluded for incomplete data,41cases were included; while the other1882cases who had been used oxygen therapy less than28days,defined as non-BPD group.(1)All these low birth weight infants were grouped to7groups according to birth weight:less than1000g,1000-1249g,1250-1499g,1500-1749g,1750-1999g,2000-2249g,2250-2499g,to calculate the incidence of BPD in each group.(2)Randomly selected41cases from the non-BPD group as control group. All clinical data,maternal and neonatal risk factors, total volume of fluid intake, caloric intake after3d、7d、10d,and vitamin A intake were compared between BPD group and control group.(3) BPD group was divided into mild,moderate and severe group according to Bancalari criteria. Comparing clinical features of two groups within1year after borth.Results:The incidence of BPD with birth weight<1000g,1000-1249g,1250-1499g,1500-1749g,1750-1999g,2000-2249g,22502499g were10.34%、16.28%、4.97%、2.77%、1.52%、1.20%、0.72%.the incidence of BPD decreased significantly with increased birth weight. Chi-square test results showed that:For BPD group,RDS, respiratory failure, apnea, pulmonary hemorrhage,critical illness levels,HB<128.5g/L, mechanical ventilation, ventilator duration>7d,PaO2/FiO2<300and patent ductus arteriosus are found higher than the non-BPD group.By multivariate logistic regression analysis,HB<128.5g/L, respiratory failure, apnea and PDA were found to be independent risk factors for BPD development.The total volume of fluid intake after3d、7d、10d,vitamin A intake were found to no significant difference between BPD and control group.while total caloric intake and average HB of BPD group during hospitalization are significantly lower than the control group.17of41cases had mild BPD while24cases had moderate or severe BPD, Gestational age <32weeks is the only risk factor for severity of BPD.(P=0.024).Conclusion:The incidence of BPD was2.59%, the incidence of BPD decreased significantly with increased birth weight.The independent risk factors for BPD development are HB<128.5g/L, respiratory failure, apnea and PDA.The total energy intake of the BPD group was significantly lower than non-BPD group.Gestational age<32weeks is the only risk factor for severity of BPD.(P=0.024).
