Objective:To explore the clinical effect of total hip replacement in the treatment of DDH with secondary osteoarthritis.Methods:From Feb.2005to Feb.2011,32patients with osteoarthritis secondary to congenital hip dysplasia underwent total hip replacement. All of the patients are female. Aging from25-55years(mean,40years). Preoperative diagnostic methods include medical history,symptoms, signs, laboratory tests and imaging exams.All of the patients complained about hip pain,shortening of the diseased limbs,deformity with limited activities. Obvious changes in blood sedimentation rate and C-reactive protein level were not noticed. Two patients’ X-ray films show bone defects, the remaining patients are all positive with osteoarthritis by medical imaging. All the patients adopted the total hip replacement, and during the following-up, Harris Scoring system is used to evaluate the hip function.Results:32patients(thirty-six hips),postoperatively,were all followed up ranging from7 months to24months,on average,14months. All patients had first intention healing, symptoms of common peroneal nerve injure occurred in two cases (healing), without any surgical incision infection, vascular injuries,early prosthesis loosening and other complications. There are no clarity lines around prosthesis in X-ray films. Clinical manifestations of prosthesis loosening were not noticed. Postoperatively, all the patients receive completely pain relief, hip mobility increase; patients with shortened limbs adopted proper lysis and extended femoral head prosthesis,and the results are satisfactory. Harris score improved from an average of42.5±1.6points back to an average of88.5+2.0points.The difference is significant.All patients could walk on their own,care for themselves,and return to normal lifeConclusion:Total hip replacement is an effective method in the treatment of the osteoarthritis secondary to congenital hip dysplasia. It not only have a remarkable in effect on odynolysis,but also can restored the normal position of the hip,increase the stability of the hip and its flexibility.Thus,making the diseased hips become the painless workable joints.Well,more attention should be paid to the developmental state of the acetabulum.Bone defects in acetabulum have to be fixed. but also And femoral head prosthesis have to be firmly implanted in the marrow cavity with femoral shaft pressed tightly,meanwhile,pay attention to the full correction for the femur rotating deformity.
