BackgroundWith the rapid socio-economic development, career women increasingly became a large group, and they played a vital role in the socio-economic development,the position of women had been greatly improved and their health status was paid more and more attention to by the society and its concerns. Due to the particularity of the railway enterprise, the working conditions (continuous operation, the normal biological rhythm interrupted, etc.) limit, there is a high incidence of some diseases. Through a series of literature search, I found a lot of the analysis of examination results of female workers of the railway, but most of them just assessed the reproductive health status of female workers, analyzed the risk factors and came up with simple suggestions. But the proposal is still a lack of a comprehensive analysis of examination data, such as the occurrence of chronic non-communicable diseases in women workers, the strength of association between gynecological diseases of female workers with a variety of risk factors, the same risk factors for different diseases and the pathogenic role of size. There are more than30,000female workers in Jinan Railway system, Jinan region alone more than6,000people, most of whom work in the trains, stations and other front-line units.Therefore, it is necessary to investigate the health status of female workers of the railway system in this particular industry survey, to analyze the relative between risk factors and disease and propose appropriate health promotion programs.ObjectivesThrough analysis of the health status of the railway female workers in Ji’nan, to understand the disease distribution and severity of impairing female workers and influnencing factors of disease; explore measures to promote the health of female workers, which can offer a basis for improving their health level, formulating the disease early preventive measures and labor protection counter measures.MethodsThis study employs the census approach of the cross-sectional study. Health examinations were made for working married female workers in Jinan railway system, and then relevant data and information were collected. Data received by the verification were checked accurate and then were input into the computer, Tcreate an Excel database, with SPSS17.0Software statistical processing of data. The statistical methods include:descriptive statistics, chi-square test, non-conditional logistic regression analysis. a=0.05.Results1.There were6848objects in this study, and actually6164people took the health examination,while684made no response.The response rate was90.01%, and20were removed for the incomplete data, so there were6144female workers for data analysis (Average age of40.23±8.18years, minimum age of20, maximum56-year-old).2.The major diseases (prevalence rate):uterine fibroids (20.82%), hyperplasia of mammary glands (16.36%), cervicitis (15.93%), fatty liver (13.44%), hyperlipidemia (10.95%), hypertension(8.19%), gallbladder disease (6.48%), hyperglycemia (3.52%), arrhythmia (3.09%), coronary heart disease (0.18%). The disease at the top three prevalence rate were uterine fibroids, hyperplasia of mammary glands and cervicitis.3.The prevalence of gynecological disease increases with age, peaked at46to50-year-old age group,64.17%. The prevalence of uterine fibroids increases with age,in46to50years the highest prevalence in the age group,31.69%; the prevalence of hyperplasia of mammary glands31to45-year-old age group at a higher level,46-year-old then gradually decreased; the prevalence of cervicitis, the lower of25years of age, the prevalence was1.63%, increased gradually after the age of26,31to40age group at a higher level, after gradually decreased with age.4^The prevalence of several chronic diseases (fatty liver, hyperlipidemia, hypertension, etc.) showing an upward trend with age, between age and the disease was positively correlated (P<0.01), in45to56yearsthe highest prevalence in the age group, however, certain chronic diseases (such as fatty liver, hyperlipidemia, etc.) in a certain proportion of the younger group.5. The results of multivariate analysis:①The risk factors for uterine fibroids associated with age, high diastolic blood pressure (OR=1.608,95%CI:1.142-2.265), platelet count increased (OR=1.347,95%CI:1.118-1.622), low hemoglobin (OR1.821,95%CI:1.425-2.326), during36to40years of age, the risk of uterine fibroids is4.233times the35-year-old age group(OR=4.233,5%CI:3.118-5.747), while from41to56years of age the risk of uterine fibroids is10.008times the35-year-old age group (OR=10.008,95%CI:7.692-13.021)②Risks associated with the incidence of hyperplasia of mammary glands risk factors age, type of work (OR=1.590,95%CI:1.317-1.920), low blood sugar (OR=1.600,95%CI:1.272-2.012), triglycerides (OR=1.500,95%CI:1.204-1.867),. In31to45age group the risks of hyperplasia of mammary glands are3.778times that of30years of age (OR=3.778,95%CI:2.901-4.919), when the age increases from46to56years the risk of hyperplasia of mammary glands are2.473times that of30years of age (OR=2.473,95%CI:1.855-3.298)③Cervicitis risk associated factors of age, type of work (OR=1.210,95%CI:1.019-1.437), high cholesterol (OR=0.439,95%CI:0.268-0.719), lower triglycerides (OR=1.306,95%CI:1.041-1.637), In31to45age group the risks of cervicitis incidence are2.281times that of30years of age (OR=2.281,95%CI:1.810-2.874), when the age increases from46to56years the risk of cervicitis are1.460times that of30years of age (OR=1.460,95%CI:1.126-1.892)Conclusion1. The prevalence of diseases of the top three among Jinan railway female workers were uterine fibroids, hyperplasia of mammary glands, cervicitis, with the prevalence of20.82%,16.36%,15.93%.2. Uterine fibroids, hyperplasia of mammary glands, cervicitis, and other gynecological diseases are age-related prevalence, showing an upward trend with age, during36to40years of age, the risk of uterine fibroids is4.233times the35-year-old age group, while from41to56years of age the risk of uterine fibroids is10.008times the35-year-old age group. In31to45age group the risks of hyperplasia of mammary glands and cervicitis incidence are3.778times and2.281times that of30years of age, when the age increases from46to56years the risk of hyperplasia of mammary glands and cervicitis are2.473times and1.460times that of30years of age, suggesting that age is a common and independent risk factor of studying major diseases.3.High diastolic blood pressure is an independent risk factor for uterine fibroids, increase in platelet count and low hemoglobin are related to uterine fibroids and may be the results after the onset of uterine fibroids;Blood glucose and lipid abnormalities were associated with mammary hyperplasia and cervicitis. The specific mechanisms need to be further researched and analysed.
