Purpose1.To investigate the prevalence of malnutrition in hospitalized liver cirrhosispatients and explore the relationship between nutritional status and prognosis.2.Subjective global assessment(SGA)、 Mini Nutritional Assessment (MNA) andNutritional Risk Screen2002(NRS2002) were used to identify malnutrition andnutrition risk of hospitalized liver cirrhosis patients, the relationship betweennutritional status and patients’ poor clinic prognosis and lenth of stay in hospital wereanalyzed.The Clinical value of these three nutrition evalution tools used indecompensated hospitalized liver cirrhosis patients was tested,so as to providetheoretical proof in selecting eligible hospitalized liver cirrhosis patients those whoneeded nutrition support effectively at early period of hospitalization for clinician.Methods It was a prospective study. Subjective global assessment(SGA)、MiniNutritional Assessment(MNA)and Nutritional Risk Screen2002(NRS2002) were usedto identify malnutrition and nutritional risk of200hospitalized liver cirrhosis patientsbetween May2010and May2011at the department of gastroenterology,the firstaffiliated hospital of anhui medical university,retrospectively.Patients’prognosis ofmorbidity and complications after6months was followed,CTP score and the lenth ofstay in hospital was recorded to explore the relationship between nutritional status andprognosis in hospitalized liver cirrhosis patients. The consistency among SGA、MNAand NRS2002in evaluating malnutrition and nutrition risk in hospitalized liver cirrhosispatients was also analyzed. SGA and NRS2002were used to identify malnutrition and nutrition risk of97elderly liver cirrhosis patients and103younger cases, then theclinical data of patients of two groups was compared,and the relationship betweennutrition status and CTP score、complications、length of hospital stay、clinical relativefactors were analyzed,simultaneously.Results1.Prevalence of malnutrition as defined by SGA,MNA were31.5%and85.5%,prevalence of nutrition risk as sreened by NRS-2002was53.5%. The prevalenceof malnutrition and nutritional risk was increaseing as CTP score increased.2.Theincidence of morbidity (33.33%VS10.95%,P<0.05)and complications(87.30%VS57.66%, P<0.05)in liver cirrhosis patients with malnutrition was significantly higherthan those without malnutrition,and the hospital stay extended obviously as evaluatedby SGA. When the nutrition status of liver cirrhosis patients was assessed by MNA,theincidence of complications increased remarkably(70.59%VS48.28%,P<0.05), thehospital stay extended ramarkably in patients with malnutrition (P<0.05),but thedifference in revalence of morbidity (19.30%,VS10.34%,P>0.05)between patientswith and without malnutrition was not significant.The incidence of morbidity(23.01%VS10.75%, P<0.05) and complications(79.44%VS52.69%,P<0.05) in livercirrhosis patients with nutrition risk was significantly higher than those withoutnutrition risk,the hospital stay of liver cirrhosis patients with nutrion risk extendedobviously as evaluated by NRS2002(p<0.05).3. NRS2002and SGA,k=0.435;NRS2002and MNA,k=0.221. the consistency between NRS2002and SGA was betterthan that between NRS2002and MNA.Conlusion1.The prevalence of malnutrition and nutritional risk was high;Theincidence of morbidity and complications was significantly higher in liver cirrhosispatients with malnutrition or nutrition risk than those without malnutrition or nutritionalrisk.2. Liver function damaged severely and the lenth of stay in hospital extended obviously in patients with malnutrition or nutrition risk.3.NRS2002was a feasible toolin screening decompensated hospitalized liver cirrhosis patients with nutritionalrisk,could be recommended to use in decompensated hospitalized liver cirrhosis patientsto select eligible patients those who would benefit from nutrition support.
